About this file
This is a Sample and Specification Evaluation Report for Request for Proposal No. 2000000001724 concerning motorcycle size license plate bags. The evaluation document outlines a sequential pass/fail assessment process whereby bidders' samples and documentation are evaluated against specific RFP requirements in Section 1.1, with disqualification occurring upon failure at any step. Bidders are required to submit one bundle of 50 to 100 motorcycle size license plate bags along with documentation detailing bag specifications, including thickness. The bags must be polyethylene construction measuring 2.5 mil thickness and conform to dimensions of 7.75 inches (±1/8 inch) in length by 4.5 inches (±1/8 inch) in width with an envelope configuration featuring side seal only construction.
Additional specification requirements include an overall bag length with lip of 9.625 inches (±1/8 inch) comprised of a 7.75 inch bag and 1.875 inch lip, perforations positioned 0.25 inches above the bag opening, and a perforated lip measuring 1.875 inches across the full 4.5 inch width with at least three perforations per inch evenly spaced. Each stack must include a minimum of two 1-inch plastic round fasteners through the lips and a 4-inch by 4-inch cardboard clipboard positioned on the bottom of the stack. The evaluation report includes spaces for four evaluators to document their findings and sign-off, indicating a multi-person review process for sample compliance assessment.
