Section 1: System Description and Contact Information
WELL
The Sacramento well was constructed in 1961, is 100’ deep and has the pump set at 90’. Static water level is 10’ to 35’. Casing is 5” steel including 40’ of screen at the bottom. The current pump is a 1/2 hp submersible pump installed in 2003.
The casing extends approximately 24” above grade and is surrounded by a small slab approximately 3’ x 3’.
TREATMENT, PUMPS AND OTHER EQUIPMENT
An injection pump is housed in a small pump house adjacent to the wellhead and adds a chlorine residual to the stream of water. Water is metered through a mechanical meter. The injection pump is wired to run only when the well pump motor runs. The well and all equipment is powered by the local electric utility. A sediment filter is installed after the chlorine injection pump. A mercoid pressure switch controls the well pump operation via a dedicated ¾” line between the tank and the pressure switch.
STORAGE
Water is transmitted by a two way water line approximately 700’ horizontally and 150’ vertically to a 28,000 fiberglass storage tank. Water feeds into the tank through a side connection near the bottom of the tank and is protected by an insulated enclosure. There is no level indicator on the tank.
DISTRIBUTION
Water is distributed from the storage tank by gravity via 2’ PVC pipe system to various buildings and hydrants at the administrative site. Refer to the site plan of the appendix for information on the location and layout of distribution lines and features.
SPECIAL ISSUES
This system is semi-seasonal with very limited use in the winter but water is kept on at the shop and office. The water is turned off at the yard stops for the quarters and the east end of the system can be drained. Stagnation and quality problems associated with extremely low use is can be experienced at the remaining buildings unless some volume of water is allowed to flow through the lines occasionally during the off season. The system is potentially under the influence of Surface water but has not been tested by the State.

Section 2: Bacteriological Sampling
Revised Total Coliform Rule (RTCR) Sampling Frequency Based on our population of 50 and the sample requirements provided in Tables 1 and 2 of the instructions, we are required to designate a minimum of 4 routine sample locations per month and collect a minimum of 1 routine bacteriological samples per month.
Location We are required to identify each of our routine monthly bacteriological sample locations, and the three (3) repeat sites (original, up- and downstream) associated with each routine site with either a physical address or physical location. Those addresses/physical locations are listed on the RTCR Sample Site spreadsheet in Appendix B. That spreadsheet has been submitted to the DWB according to the instructions that accompanied the spreadsheet; the submittal acknowledgement is included in Appendix B.
Appendix C includes the map(s) showing where these routine and repeat sites are located throughout our distribution system.
We understand that the DWB will be verifying that we sample from each routine and repeat sample location designated on our DSSP. We further understand that our DWB Compliance Officer will also verify that we collect the routine samples at regular intervals from month to month (i.e., same week each month) and that we are rotating through each major and minor portion of the distribution system.
Alternate Repeat Sampling Locations The RTCR Sample Site Spreadsheet in Appendix B designates repeat sample locations that are within the five (5) connections up- and downstream of the original routine sample location. However, we understand that we can use alternate up- or downstream repeat sampling locations that are outside the five (5) connections from the original sample site as long as we submit a Standard Operating Procedure (SOP) that specifies our criteria for selecting these alternate repeat sampling sites on a situational basis (i.e., for any time we are required to collect repeat distribution system samples and determine that the prescribed repeat locations do not adequately identify potential pathways of contamination).
We do not plan to use any alternate repeat sampling sites.
Groundwater Rule (GWR) Sampling One (1) Triggered Source Water sample is required to be collected from every active well if any of our routine monthly samples test positive for Total Coliform (TC) or E.Coli (EC). These Triggered Source Water Samples will be collected directly from each of our wells prior to any treatment and are

Sampling Procedure
1) Select the sampling tap.
a) A tap, such as faucet or small valve, is preferable.
b) Avoid taps with a leak at the stem or taps with a swivel joint.
c) Aerated or screened nozzles may harbor bacteria. The aerator or screen must be removed before collection of the sample.
d) Use/install a smooth nosed sample tap.
2) Place all carbon filters, sediment filters and water softeners on bypass unless operated by the public water system.
3) Sanitize the nozzle of the tap with a chlorine solution.
a) Use a 6% sodium hypochlorite solution, such as household liquid bleach. Do not use chlorine solutions with special scents. To prepare a sanitizing solution, add one ounce of bleach to one gallon of water (or 1 tablespoon per half-gallon). Store the mixed solution in a tightly closed screw-capped container. The solution should be discarded and remade 6 months after preparation. Stronger solutions can be used; however, some faucet discoloration may result.
b) Flush the sample tap to waste for 1 minute. Close the valve.
c) Apply the sanitizing solution (prepared in step a.) to the nozzle.
This can be accomplished by either using a spray bottle or a plastic bag.

i) Using a spray bottle, saturate the tap opening with sanitizing solution then wait at least 2 minutes before proceeding, or
ii) Place a bag over the nozzle and hold the top of the bag tightly on the tap.
Alternately squeeze and release the bag to flush the solution in and out of the tap. Do this for 2 minutes. A fresh solution and bag must be used to sanitize each tap.
4) Flush the tap.
(a) The sample to be collected is intended to be representative of the water in the main. The tap must be opened fully and the water run to waste for at least 3-5 minutes to allow for adequate flushing of the piping between the tap and water main.
5) Reduce the flow from the tap to the width of a pencil to allow the sample bottle to be filled without splashing.
6) Open the sample bottle.
a) Grasp the bottom of the same bottle.
b) Remove the cap and hold the exterior of the cap between your fingers while filling the sample bottle. Do not lay the cap down. Take care to not touch the mouth of the sample bottle or the inside of the cap with fingers as the sample could become contaminated.
c) T h e sample bottle must be open only during the collection of the sample.
7) Fill the sample bottle.
a) Do not rinse out the sample bottle before collecting the sample. Do not remove any pills, powder, or liquid from the sample bottle. The sample bottle contains a small amount of sodium thiosulfate to neutralize any chlorine in the water.
b) Do not touch the rim or mouth of the sample bottle during collection of the sample.
c) Do not overfill the sample bottle. Fill the sample bottle to within ½” to 1” of the top or to the indicator line on the sample bottle.
8) Immediately recap the sample bottle tightly.
9) If there is any question as to whether a sample has become contaminated during collection, it must be discarded and a new one collected in a new sample bottle.
10) Deliver the sample to the laboratory as soon as possible.
11) Samples should be kept cool after collection and during transport to the laboratory. The laboratory must receive the sample so that analysis can be initiated within 30 hours after collection. Allow the laboratory adequate time to analyze the sample. Certified laboratories will not test samples greater than 30 hours old because the results will be invalid.
Additional information A bacteriological sample report form is supplied with each sample bottle. The top half of the form is to be filled out in a legible manner using an indelible pen, rubber stamp, or typewriter. Do not use a fountain pen or other pens having water soluble ink.
Samples must be collected in sample bottles supplied by the certified laboratory.

Bacteriological sample report forms that have not been properly completed, including the name of the water system, PWS ID#, address, date and time of collection, sample type and location (specific tap) and signature of collector will not be accepted for bacteriological examination.
Sampling Requirements New Mexico Regulations require that a certified sampler or certified operator collect the RTCR and GWR samples. Because of this requirement, our certified operator will be required to collect our bacteriological samples. Once collected, our operator will submit the samples and their Chain-of-Custody (CoC) forms to the following certified laboratory within 24 hours of the sample being collected:

Our water system triggers an assessment with the RTCR if:
We get 2 or more TC+ samples in any one (1) month (for systems that take <40 samples/month)
We fail to take all the required repeat samples Any one (1) of these conditions will trigger a required Level 1 assessment/correction action
A Level 2 assessment/corrective action is triggered if we get:
An EC Maximum Contaminant Level (MCL) violation; or An EC monitoring violation; or We trigger two (2) Level 1 assessments within a rolling 12 month period
GWR
We are in compliance with the GWR if our Triggered Source Water sample(s) are free of EC.
We will immediately notify our DWB CO if any of our Triggered Source Water samples test positive for TC or EC. At that time, we can be required to conduct additional sampling, correct significant deficiencies, or disinfect our water to meet 4-log treatment requirements.

Section 3: Disinfectant Residual Monitoring
Frequency We are a chlorinated system, and as such we are required to measure chlorine residuals at the same time we collect our monthly routine RTCR samples.
Based on our population of 50 and the requirements provided in Tables 1 and 2 of the DSSP template instructions, we are required to designate a minimum of 4 chlorine residual monitoring locations per month.
Location Chlorine residuals are measured at the same time and from the same locations where we collect our routine monthly RTCR samples. Results are recorded on each bacteriological CoC form and submitted to the lab with those samples. The sites were chosen based on the fact that they are representative of the entire distribution system, and are designated on the map(s) included in Appendix C.
We also measure chlorine residuals at the 1 chlorine residual monitoring sites throughout the month as part of our best management practices. This is to ensure that injection dosages are sufficient to meet chlorine demand and maintain adequate residuals in the entire distribution system, including vulnerable portions of the system. Vulnerable areas are anywhere we might have increased water age or stagnant water (storage tanks, high elevation/low pressure, low occupancy, dead ends). We use these chlorine residual results along with other information to focus our best management practices such as line and hydrant flushing (along with valve exercising).
Methodology and Reporting Our certified operator uses disposable latex gloves and color-wheel test kit or digital colorimeter to measure chlorine residuals and follows all sample collection, handling, measuring and equipment calibration protocol specified in the operation manual.
All chlorine residuals that are measured during RTCR routine and repeat sampling and for best management practices. These results are required to be recorded on the bacteriological chain of custody forms that are submitted to the lab and are also recorded on the Residual Disinfectant Residual Measurement Sampling Report (Appendix E).

Section 4: Distribution System Asbestos Sampling
Frequency We have reviewed our sample schedule on Drinking Water Watch (included in Appendix A) and verified that we are not required to sample our distribution system for asbestos.

Section 5: Entry Point (EP) Chemical Compliance Sampling for Organics, Inorganics & Radiologicals
Frequency Chemical samples are collected at a time frame and frequency that is established by the DWB. We keep track of our sampling schedules (Appendix A) for all SDWA primary drinking water contaminants using the DWB Drinking Water Watch website.
Location These chemical compliance samples are required to be taken at the Entry Point (EP) to the distribution system, regulatory defined as where potable water is first made available to our customers. Our EP sample point is a water hose bibb is before the chlorinator as shown in the following photograph:
Sampling Method We are a federal facility and NOT subject to Conservation Fund payments.
Consequently, we are responsible for our own EP chemical compliance sampling according to the DWB sample schedule (Appendix A). Our certified operator collects our EP chemical compliance samples and submits the samples and completed CoC forms to the following appropriate certified laboratory(s) for analysis:

Compliance Our water system is in compliance if the EP chemical compliance samples are collected according to schedule and chemical concentrations meet all the MCL requirements set forth by the SDWA primary drinking water standards.
We are required to, and will notify our DWB CO of any violations and follow all Public Notification Rule and other regulatory requirements in the event of any MCL, sampling or reporting violations.
