STATEMENT OF WORK
Project #523A4-CSI-401
Project Title: Cath Lab Site Prep
1.0 GENERAL:
The Design-Build Contractor shall provide Architect/Engineer professional services and construction services for site preparation for the installation of new equipment in Cath Lab of Building #1 at the VA Medical Center (VAMC) West Roxbury Division.
This project will provide construction of utility services and finishes necessary in the designated locations to allow for the installation of an image-guided therapy system by the Equipment Contractor. The work shall include all labor, transportation, materials, apparatus, tools, equipment and permits necessary to provide for the renovation of the room, removal of the existing flooring, ceiling, sinks, casework, doors and installation of new flooring, ceiling, casework, doors, sinks, lighting, painting, patient lift system, med gas, lead line divider and relocation of electrical work. Work to be performed in accordance with the Statement of Work, Specifications, Drawings, Federal, State, and Local Codes.
Strict noise and infection control measures shall be followed. Existing life safety systems within Building 1 shall not be impaired at any time during the performance of the work.

2.0 PROJECT SCOPE OF WORK:
1. After award, but prior to the start of any construction, the Design-Build Contractor shall contact the VA COR to review proposed methods and plans for completion of work and to review infection control procedures, interim life safety procedures, and the contractor’s site-specific safety plan. Field work shall not commence until these plans have been submitted, and approved as detailed below.
2. General Contractor’s employees and subcontractor employees shall not enter the VA Medical Center site or project site without appropriate VA issued PIV badge. See specification 01 00 00 subpar 1.5 Construction Security Requirements for more full requirements. This can be found on the VA Technical Information Library (TIL) at https://www.cfm.va.gov/til/.
3. General Contractor shall furnish and post a board where at the job site that includes a copy of Rates and Wages, the Davis-Bacon poster and schedules, ILSM/ICRA/GEMS/Patient Safety

4. The GC shall not proceed with mobilization or construction until after Veteran Affairs’ acceptance of all required submittals and coordination drawings.
5. The GC shall submit a Site Superintendent Designation Letter designating the Site Superintendent to be dedicated to this project only. The Site Superintendent shall be physically present at the construction site (Building 1, Fifth Floor) at all times. The GC shall submit, via e-mail, a list identifying key GC personnel, their responsibilities, contact info and 24 hour/7 day per week primary and secondary contacts. Superintendent Communications: At all times during the performance of this contract, the GC’s designated Site Superintendent is to be available by cell phone. After-hours phone number shall be posted at the construction site on the required board in item #2 above.
6. Before placement and installation of work subject to acceptance checks and tests, the GC shall coordinate and notify the VA Project Engineer (COR) in sufficient time to enable testing personnel to be present at the site in time for proper testing and field inspection. This includes notification prior to closing walls, ceilings, chases, etc. Such prior notice shall be not less than two (2) weeks unless otherwise designated by the VA Project Engineer (COR).
7. The Design-Build Contractor shall provide the following services for the design, documentation, construction, and construction administration for the preparation of installing image-guided therapy system by the Equipment Contractor.
8. The design and construction are anticipated to be non-concurrent. The starting of construction is depended on the completion of design and conditions are met in item #1 of this section.
2.1 DESIGN SERVICES:
A. Basic Services: The following tasks shall be performed:
Provide the services of a design professional that is registered and licensed in the various design disciplines necessary for the work required, to prepare construction documents and to provide construction support services as further described in this Scope of Work.
Provide conceptual designs for review by the West Roxbury Division staff to determine the appropriate functional arrangement of the space designated for installation of the new equipment. Revise conceptual designs as required to address comments by West Roxbury Division staff. One conceptual design will be accepted by West Roxbury Division staff as the basis for design development.
The contractor’s hired design professional plans for and completes the necessary work required for Section 106 coordination during designs. Design Professional shall provide Section 106 compliance report along with the design drawings. Contractor and design team are responsible to submit all necessary drawings and completed form to authority.
Incorporate all items of work required or recommended by the equipment manufacturer for the successful installation and operation of the equipment by the Equipment Contractor.
Provide architectural design to meet the needs of the proposed use and in accordance with the accepted conceptual design.
Conduct a structural survey and investigation of the project location to the extent necessary to determine the ability of the existing building structure to support all loads imposed by the work of this project. The structural survey and investigation shall be conducted by a qualified structural engineer. The structural engineer shall determine whether the existing structure can support the new equipment. If the structural engineer determines that the existing structure can support all loads imposed by the work of this project and that no supplemental framing and/or reinforcement is necessary, then the structural engineer shall provide a written certification to that effect.
Conduct a survey and perform a study of the existing normal and emergency electrical power distribution systems to determine whether sufficient electrical capacity is available to meet the needs of the proposed use. Determine needs for normal and emergency electrical power. All electrical power needs shall be determined by a qualified electrical engineer and the design of electrical work shall be prepared by a qualified electrical engineer.
Conduct a survey and investigation of the mechanical, plumbing, and fire protection systems of Building 1 to the extent necessary to determine their available capacities and the ability of these existing systems to meet the needs of the proposed use. The existing fire alarm system is EST-3 as assembled and bench tested by the manufacturer of record Signet Electronics Systems, Inc.
Investigate the existing HVAC system and determine if the existing system will meet the need of the proposed use. HVAC needs shall be determined by a qualified mechanical engineer and the design of HVAC work, if any, shall be prepared by a qualified mechanical engineer. HVAC work shall tie into the existing building management system (TRANE). The existing BMS system for West Roxbury is TRANE Ensemble. Upgrade existing controls as needed.
Provide telecommunications and data connection devices to suit the needs of the proposed use. Telecommunications and data work shall be performed in conformance with West Roxbury Division requirements and standards.
Provide all necessary engineering and design for plumbing, fire protection, electrical systems, alarms, security, radiation shielding, architectural, structural, and all other work to meet the needs of the proposed use and for a complete and usable installation. Radiation shielding requirements will be developed and tested by a West Roxbury Health Physicist.
Prepare construction documents for the proposed work. Documents shall consist of construction drawings and technical specifications and shall be prepared in sufficient detail to fully describe the extent of work required for a complete and fully functional installation and to demonstrate compliance with all VA Standards and contract requirements.
Provide construction period services by the Design-Build Contractor’s design professional, consisting of review of product and material submittals and shop drawings, respond to inquiries for clarification of construction documents prepared by the design professional, review change order proposals when requested by the VA, and prepare as-built drawings upon completion of the construction.
Provide 2 x site visits by the Design-Build Contractor’s design professional during construction and provide one final inspection of the completed work by the Design- Build Contractor’s design professional to determine whether the work was completed in accordance with the approved construction documents.
The design shall include radiation shielding divider.
Structural Engineering Design Services: If the investigations by the Design- Build
Contractor’s structural engineer determine that the existing structure cannot support the new work, the Design-Build Contractor shall provide design of whatever supplemental framing and/or reinforcement is necessary to carry the weight of the new equipment.
Electrical Engineering Design Services: If the investigations by the Design- Build Contractor’s electrical engineer determine that either the existing normal or emergency electrical power systems, or both, are insufficient to meet the power needs of the new work, the Design-Build Contractor shall provide design of whatever improvements to the existing electrical power systems are necessary for the proposed use.
Mechanical Engineering Design Services: If the investigations by the Design- Build Contractor’s mechanical engineer determine the existing HVAC system is insufficient to meet the cooling requirements of the equipment, the Design-Build Contractor shall provide design of whatever improvements to the existing or new mechanical systems are needed to meet the requirements of the equipment.
Provide third party commission service for the new electrical and HVAC equipment installed.
B. The Design-Build Contractor shall be responsible for the entire design of this project, including confirmation of existing conditions. The Design-Build Contractor shall also be responsible for developing a detailed infection control plan for the construction work. This plan shall be based on medical center infection control policies and will be site specific for the areas involved in the project. The design must include a phasing plan that minimizes any negative effects to the normal operations of the medical center and this building.
2.3 CONSTRUCTION SERVICES:
Provide all construction of site preparation as shown on the construction documents approved by VA West Roxbury Division and in accordance with the instructions and recommendations of the equipment manufacturer. The VA medical equipment vendor is responsible to remove existing equipment and install new Cath lab equipment. Contractor will perform the balance of the work to include flooring, ceiling, drywall, case work, structural modifications to the building, upgrades to MEP systems, med gas, and installation of radiation shielding divider as needed. All work by the Contractor must be in conjunction with the VA equipment vendors equipment requirements and any work that is related to Cath Lab removal/installation and not performed by Vendor is sole responsibility of contractor.
General Requirements Provide full-time supervision to coordinate safety and quality control, material storage and handling, maintain project records, and control all construction activity onsite.
Provide temporary barriers, negative air pressure machine with readable monitor, and dust control as needed.
Perform daily cleanup of the jobsite, temporary debris storage and removal from the jobsite, and final cleanup prior to occupancy.
All demolition and installation shall not impact nearby EP lab and Cath Lab operation. Off hour work shall be considered for some of work that may interrupt surrounding area during normal hours.
Demolition - restructuring and renovation of the existing spaces (5D110, 5D110A, 5D109 and 5D109A) including med gas upgrades, electrical upgrades, lighting upgrades, existing finishes (floors, walls, and ceilings), and removal of existing HVAC duct work and grill as needed.
Demo floor tiles and existing walls as needed for reconfiguration of the rooms.
Reconfigure existing ductwork and grills.
Remove existing raceways, and related electric devices/components for existing equipment.
Make safe for removal and disposal of the existing UPS if the existing UPS is deemed not compatible to new equipment.
Removal of injector and turning it over to VA Personnel.
Remove and replace the following items within the limit of work: doors, windows, door hardware, floors, ceiling tiles and lights, control room window, DDC, sprinklers, sinks and fixtures, case work, counters, med gas, storage cabinets/spaces, light switches, electrical outlets, ductwork, bathroom plumbing and fixtures, patient lift and any medical devices/accessories (as needed), diffusers, fan coils, raceway and conductors, accessories, and telephone/data jacks.
Metals Contractor to provide and install new ceiling patient lift.
Contractor shall install backing plates and support structures for equipment in accordance with the manufacturer.
Wood and Plastic Provide in-wall blocking for the drywall assemblies. Remove and replace existing counter within the work limit.
Provide new casework.
Provide pulls and knobs in nickel finish.
Shelves shall support 25-pound load.
All cabinet doors shall have damped easy pull hinges.
Casework shall be performance duty level 2.
Doors & Windows The Contractor to evaluate condition of the door and hardware, replace as needed. Hardware shall be Corbin Russwin. All locks shall be Corbin Russwin with 6 pin removable core.
Evaluate window condition and replace new as needed.
Finishes
Provide new drywall, moisture resistant and finish for existing furred and partitions to modify the spaces as per the 100% design drawings. Existing framing may change as per the 100% design drawings.
Provide a new white 15/16” T-bar ceiling grid and 2x2 rectangular white gypsum-based ceiling tiles, as required within the project limit.
Provide and install new flooring.
Prime all walls and provide two-coat latex paint finish.
Provide 6-inch vinyl wall base throughout.
Fire Suppression & Fire alarm System & Communications & Electrical The Contractor shall modify the layout of the existing wet sprinkler system as designed by
Engineer of Record. Flexible connectors are prohibited. Fire alarm system modification as required and as per the 100% design drawings. The contractor shall provide communication and IT systems and wiring; detection and alarm systems and wiring; access and surveillance systems and wiring; and automation and control systems and wiring. The contractor is responsible for removing and reinstalling existing smoke detectors connected to existing systems, and where the detectors are to be removed with finish demolition.
Provide electrical work specified by equipment manufacturer. Provide power connections, outlets, terminal boxes, cables, conduit, pull boxes, controls, switches, conduits and raceways, and devices as required for the installation of the new image-guided therapy system.
Assist equipment manufacturer to install all wiring required for the new equipment.
Provide data connection means at the designated control booth and other locations as required in accordance with the standards of the West Roxbury Division. Installation of both telephone and data must be complete to include all cabling and connections. Work must conform to West Roxbury Cabling and Quad Drop specifications that can be found in the TIL.
Termination into the local IT closet and testing of all telephone and data systems is the responsibility of the contractor. The general contractor shall bear the cost of employing the services of E2 Telecom for final connections and testing.
Conduit, wiring, connections, and programming to the existing BMS are the responsibility of the contractor. TRANE is the existing BMS, no exceptions.
Modifications to the existing Fire Alarm system will require fire protection engineering redesign and support using licensed professionals. Existing fire alarm system “as built” or “shop drawings” will be verified by Design-Build A/E against actual system assembly prior to any redesign modification. All modifications will require updated drawings to be delivered to the VA after testing and commissioning has been completed. All work performed must meet current; Healthcare, National Fire Protection Association (NFPA), and the Joint Commission standards. The existing fire alarm system is EST-3 as assembled and bench tested by the manufacturer of record Signet Electronics Systems, Inc.
Provide service to install new UPS or reconnect as needed.
Complete Arc Flash Study for new panels as required.
Heating, Ventilation, and Air Conditioning
Reconfigure HVAC as needed.
Provide and install with new supply, return ductwork, registers, VAV box, diffusers and grilles etc. as designed with cooling and heating capacity for the designated work limit.
Provide an AABC or NEBB certified TAB company to perform pre and post-air balancing studies to verify and address treatment room airflow as needed.
Provide direct digital control (DDC) for all HVAC equipment. DDC shall be by Trane Ensemble. Equipment shall control via space temperature and space humidity. Provide unit with full DDC control via the Trane Ensemble front end (start/stop/status, freeze control, discharge air temperature, chilled water temperature, hot water temperature, power on/off, smoke detector shutdown, speed control, alarm reset, valve position and temperature control).
Supply and return ductwork shall be fully insulated with 2-inch fiberglass insulation with full Kraft face barrier.
Specialty The Contractor to provide and install radiation protection divider.
The Contractor to access and repair radiation protection within project boundary as needed.
Provide and install new med gas.
3.0 SUBMITTALS:
Design Submissions-
The Design-Build Contractor shall retain file copies of all items forwarded to the Contracting Officer and the COR under various design review submissions. Intent of such a requirement is to save time by resolving questions over the telephone, to ensure that copies are available in the event of loss or damage to the originals during reproduction, and to facilitate rework of documents for amendments made necessary by discovery of problems during later stages of project development. The same official of the Design-Build Contractor with whom the contract was negotiated shall sign all correspondence with the Contracting Officer and COR. All submissions (drawings, specifications, cost estimates, design analysis, etc.) shall be legibly identified as the percent (%) of design submission.
Each submission shall incorporate the comments made by the VA at the prior review stage.
Note that there is a limitation of 6% of construction cost, imposed by law, for the preparation of contract drawings and specifications per 48 CFR, Chapter 8, and Part 836.606-71.
a. Drawings: Base all original drawings on standard VA guidelines. Drawings shall be done in plan view at a minimum scale of 1/8" = 1 '0", where necessary, and generally 1/4" = 1'0" or larger when possible to fit on sheet. Details shall have a minimum scale of 1/4" = 1'0", and if possible, it is preferred larger. All details, title blocks, base plans, etc., shall be drawing as "XREFS" bound to the drawing for submission purposes and final issue only. All other usage shall be drawing from a common Master Set, daily if required, from a source maintained on the architect’s master database. The database shall be available by modem to all design disciplines and the VA. The purpose of this requirement is to ensure consistency and to eliminate design conflicts by guaranteeing that common elements of the design stay throughout the design life of the project.
Drawings shall be independent for architectural, mechanical, electrical, structural, civil, and any other design disciplines for the project, and shall show, on separate views, required demolition and reconstruction details for each function.
Drawings shall be prepared in accordance with United States National CAD Standard and VHA Program Guide 18-3, VHA National CAD Standard Application Guide. Electronic version of drawings shall be submitted using IBM compatible "AutoCad” and in Portable Document Format (.pdf). Final drawings shall be plotted in ink on 30" x 42" polyester film sheets. Sepias are not acceptable as original drawings. Final drawings submitted by the Design-Build Contractor shall be originals, shall be the result of Design-Build Contractor field investigation, and shall not be direct takeoffs or tracings of any drawing provided as information.
Final drawings shall bear the VA title block and nomenclature on polyester sheets as are the sample sheets supplied by the VA. All drawings in a project set shall be on the same type and size sheets. All drawings are required to show existing conditions as of the date of the design submission.
All contract drawings shall be produced with a view to the use of half-size reductions.
Pertinent criteria to assure this result are as follows:
1. Drawings shall provide uniform photographic quality.
2. Minimum lettering height shall be not less than 1/8 inch.
3. Legends and symbols as well as size, spacing and location of notes, dimensions, etc.
shall be such as to produce legible half-size reproductions.
4. No lettering, shading or marking on the reverse side is acceptable.
5. Clearly distinguish between new work and existing work.
6. Graphic scales shall be provided on all drawings.
b. Specifications: Provide complete project specifications, to include all sections of CSI Divisions
1-50 that, in the opinion of the Design-Build Contractor, are necessary to properly and adequately construct the project by a construction contractor. Specifications shall be based on the VA Master Construction Specifications. Specifications shall be prepared in MS Word, IBM compatible format. The Design-Build Contractor shall review and edit the applicable sections of the specifications as to completeness, relevance, and correctness. The intent is to provide specifications that are suitable for the project intention and detailed enough to enable the contractor to perform the work in the best and simplest possible manner. It is equally important that the Design-Build Contractor "edit out" portions of the specifications that add additional cost to the project without additional value or quality. Not every portion of the master specification is appropriate to this particular project. Where choice of items is referenced in the specification section, the Design-Build Contractor shall consult with VAMC Facilities Management Service for direction on which option to select. The contractor shall not be provided options or choices to select from but must be provided the specific selection. The Design-Build Contractor shall obtain from Facilities Management Service the name and type of specialty materials and equipment that are required for use on the project. All items shown in the specifications shall, if named specifically by brand or production name, carry the statement "or approved equal". Exception may occur due to previously purchased or installed items interacting with new construction. In this situation, items used should be the same as existing or matched accordingly. The VA shall provide information on such items. The Design- Build Contractor shall specify construction in detail in all the specification sections. Tolerances, and workmanship, shall not be allowed to default to "common industry/construction practices”, or any similar vague standard. The legal precedence of the specifications over the drawings shall not be used to resolve design ambiguities between the drawings or notes necessary for clarity. Any contradiction, ambiguity, or statements subject to various interpretations shall be immediately corrected to reflect a single clear intention.
c. Minimum Requirements for A/E Submissions: The cost of reproduction of all review materials is included in this contract.
1. CONSTRUCTION DOCUMENT SUBMISSION (30%). The following quantities of design review material are required:
a) 3 sets of Plans (Blueline 2-full and 1-half)
b) 2 sets of Construction Cost Estimate
c) 2 sets of marked-up Sections of the VA Master Construction Specifications
d) 1 Electronic Media (CD ROM) copy of the above documents

3. CONSTRUCTION DOCUMENT SUBMISSION (60%). The following quantities of design review material are required:
e) 3 sets of Plans (Blueline 2-full and 1-half)
f) 2 sets of Construction Cost Estimate
g) 2 sets of marked-up Sections of the VA Master Construction Specifications
h) 1 Electronic Media (CD ROM) copy of the above documents
4. CONSTRUCTION DOCUMENT SUBMISSION (100%): The following quantities of design review material are required:
a) 3 sets of Plans (Blueline 2-full and 1-half)
b) 2 sets of Construction Cost Estimate
c) 2 sets of Construction Specifications
d) 1 Electronic Media (CD ROM) copy of the above documents
e) 1 copy of responses to VA comments of previous submission
5. CONSTRUCTION DOCUMENT SUBMISSION (Final): Upon completion of all revisions, submit the following items:
a) 2 copies of the final project documents on Electronic Media (CD ROM), with all project documents recorded on a single CD ROM, and each copy containing the following:
drawings on IBM compatible latest version "AutoCad” and in Portable Document Format (pdf) the final specifications in Microsoft Word format the final specifications assembled into a single file in Portable Document
Format (pdf) final construction cost estimate in Microsoft Excel Final Design Analysis in a single file in Portable Document Format (pdf).
b) 3 sets of Plans (Blueline- full 2-full and 1-half)
c) 1 set of Original Reproducible Plans with seal of the design professional on each sheet
d) 1 set of Original Reproducible Specifications
e) 1 copy of responses to VA comments of previous submission
d. Delivery Media and Format for Final Documents-
1. Format: CAD Drawings shall be prepared in accordance with VHA Program Guide 18-3, VHA National CAD Standard Application Guide. A copy of all data and files developed under this project shall be delivered to the Government in digital format. The data files should be provided on compact disk (CD ROM) in ISO 9660 format. A “Readme.txt” file must be included with the delivered digital media that includes normal transmittal information.
2. Any CDs used to store and communicate VA data files will be required to be encrypted and scanned for viruses by the VA.
3. Label: The external label for each digital media shall contain the following information:
a) Contract Number and date.
b) Format and version of operating system software, name and version of utility software used for preparation and copying files to media.
c) Sequence number of digital media.
d) List of file names on the digital media
4. Quality Assurance/Quality Control: Before a file is placed on the delivery digital media the following shall be performed:
a) Remove all extraneous graphics outside of the border area and set the active parameters to a standard setting.
b) Check to ensure that all reference files are attached without device or directory specifications.
c) All digital files shall be delivered to the Government uncompressed.
d) Include all files both graphic and non-graphic required for the project (i.e. color tables, pen tables, font libraries, symbol libraries, user command files, plot configuration files, AML plot routines, etc.).
e) Make sure that all support files such as those listed above are in the same directory and those references to those files do not include device or directory specifications.
f) Include any standard sheets (i.e. abbreviation sheets, standard symbol sheets, etc.) necessary for a complete project.
g) Document any fonts, tables, symbols, cells/blocks, line styles/types, details, reference drawings, etc., developed by the contractor, or not provided among the Government furnished materials. The contractor shall obtain Government approval before using anything other than the Government standards.
h) Each finished CADD drawing or map shall have its own separate plot file. The plot file for each finished drawing shall be provided to the Government in a specified plotter configuration.
e. Ownership: All electronic files created and reimbursed for this project shall be the property of the Department of Veterans Affairs. Contractor is responsible for the storage and security of all hard copy drawings and to be turned over at each design phase.
Prior to commencing any construction work, the Design-Build Contractor shall submit to and receive approval of the VA West Roxbury Division for the following:
Construction Submittals-
The contractor shall submit product samples, manufacturers’ literature, and data as required per the project specifications. In addition, the contractor shall pay specific attention to ensure timely submission of all product samples, manufacturers’ literature, and data covering the materials that will be installed in this project.
The contractor shall submit one (1) copy of each submittal for approval prior to installation of the related equipment. Materials shall not be ordered or installed prior to approval of the submittals. Direct all submittals to the designated COR.
Certification from the Contractor/Installer that the system specified meets all identified code and insurance requirements as required by the Government.
Material Safety Data Sheets (MSDS) for all materials to be used in the construction of the work.
The GC shall follow the VHA BHS West Roxbury Project’s submittal process attached to specification section 01 33 23. The GC shall supply a master submittal list to track all required submittal items and provide the VA Project Engineer (COR) an updated master submittal list weekly until the submittal process is completed. All submittals shall include appropriate project information, GC signature, adequate descriptive literature, catalog cuts, shop drawings, and other data necessary for the Government to ascertain that the proposed equipment and materials comply with the contract document requirements.
Catalog cuts submitted for approval shall be legible and clearly identify equipment being submitted (i.e. highlight, cloud, etc.). It is the GC’s responsibility to provide submittals that meet all necessary Federal, State, and Local code requirements in order to be approved.
Failure to meet these requirements are the responsibility of the contractor and do not justify a request of time extension.
Upon completion of the construction work and before submitting invoice for final payment, provide the VA West Roxbury Division the following:
Operation and Maintenance Manuals for new equipment Test Reports if applicable As-built drawings PDF and Mylar Arc flash study and report
4.0 CODE REQUIREMENTS:
All materials and workmanship shall comply with the codes and standards adopted by the VA.
a. OSHA
b. NFPA Life Safety Code (NFPA 101), latest edition
c. International Building Code, latest edition
d. National Standard Plumbing Code, latest edition
e. All applicable Department of Veterans Affairs (VA) master design specifications and master construction specifications (TIL)
f. West Roxbury VAMC Cabling and Quad Drop Specifications
g. National Electrical Code, latest edition.
h. EIA/TIA Standards
Note that where different specifications require different considerations for the same item of work, the more restrictive specification will generally have precedence and will be followed regardless of the specification publisher.
All materials and workmanship shall comply with the codes and standards adopted by the VA. All work shall be done in a first-class manner according to the best trade practices and to the satisfaction of the COR.
5.0 PLANNING INFORMATION AND STANDARDS:
The applicable portions of Government criteria in conjunction with industry standards shall govern the design and preparation of contract drawings, specifications, cost estimate, and design analysis.
The design shall also satisfy applicable State of Massachusetts criteria. Particular emphasis is placed on the following documents that establish minimum requirements:
1. VA Design Guides
2. VA Design Manual
3. International Building Code (latest edition), the National Standard Plumbing Code (latest edition), the National Electrical Code (latest edition), and other codes and standards cited in VA criteria, standards, and similar documents.
4. United States National CAD Standard and VHA Program Guide 18-3, VHA National CAD Standard Application Guide
5. VHA Program Guide 18-4, Design and Construction Procedures
6. The Illuminating Engineering Society of North America (IESNA) Handbook
7. VA Manual for Preparation of Cost Estimates for Hospital Projects
8. The project shall incorporate features that meet VA standards for the dignity, security and privacy of patients who utilize the facilities constructed by this project. Minimum standards are provided in Appendix A of VHA Handbook 1330.01, Health Care Services for Women Veterans.
9. Guidelines for Design and Construction of Health Care Facilities, latest edition published by the American Institute of Architects
The Design-Build Contractor or A/E shall obtain all data necessary to prepare the design documents required. The Design-Build Contractor shall confirm all existing conditions and field verify any document provided by the VA. The Design-Build Contractor or A/E is wholly responsible for both the accuracy and the level of completeness of the final product and failure to verify information is not acceptable. Information (drawings and other documentation) provided to the Design- Build Contractor or A/E shall be used for reference only.
6.0 DESIGN SITE VISITS AND MEETINGS-
The Design-Build Contractor and A/E shall visit the proposed project site to obtain the details and specifics of the relevant existing field conditions in order to fully complete the work of this project without omission. Such visits shall include a thorough inspection of normally enclosed or inaccessible areas such as spaces above the ceilings, within walls or chases, in pipe crawl spaces, and in or under basement floors. Where necessary, Design-Build Contractor or A/E shall arrange to have walls or ceilings opened at no additional expense to the Government, to examine the interior or hidden conditions to provide complete drawings. Arrangements for visits to make inspections shall be made by contacting Facilities Management Service at least 48 hours before the visit.
The Design-Build Contractor and A/E shall attend meetings at the time and place requested by the Contracting Officer and the COR. Minutes of the meetings shall be taken by the Design-Build Contractor and submitted to the Contracting Officer and COR. Meetings shall include the following:
a. Pre-Design Meeting - The Design-Build Contractor andr A/E shall meet with the Contracting Officer, COR, and, if applicable, the Using Service's Representative, to determine project requirements, the arrangement of the areas, and to clarify the scope of work.
b. Design Review Meetings-The Design-Build Contractor and A/E shall attend meetings with the Contracting Officer, COR, and the Using Service's Representative during field investigations and for review conferences as needed to advance progress of the work. As a minimum, attend one review conference after submission of the 100% documents.
c. Attend one Pre-Construction meeting.
7.0 ANSWERING INQUIRIES & ISSUING AMENDMENTS:
The Design-Build Contractor or A/E shall provide prompt answers and recommendations to inquiries made by the Government. In the event of error or omission, the Design-Build Contractor or A/E shall issue design amendments to the project documents to clarify the documents. Those amendments required to correct omissions or errors made by the Design-Build Contractor shall be at no additional cost to the Government.
8.0 WARRANTIES / PROJECT CLOSEOUT REQUIRMENTS:
The Design-Build Contractor shall guarantee all provided materials, equipment, and workmanship for a period of one (1) year from the Date of Acceptance of the entire project. During the term of the guarantee, the Design-Build Contractor shall repair or replace all defective material or equipment within 72-hours of notice without additional cost to the Government.
When the work is completed, the Design-Build Contractor shall provide one copy of as-built drawings that indicate the changes made during construction from the final design documents.
The Design-Build Contractor shall also provide operation and maintenance manuals for any equipment installed by the Design-Build Contractor for which the manufacturer recommends periodic maintenance.
9.0 PROPOSED SCHEDULE:
Work shall commence upon receipt of a Notice to Proceed from the government and shall be completed within a total of 310 calendar days after receipt of Notice to Proceed.

10.0 UTILITY SERVICES:
Utilities Services: Maintain existing utility services for Medical Center at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uninterrupted services. Where necessary to cut existing water, steam, gases, sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (including telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by the COR.
1. No utility service such as water, gas, steam, sewers or electricity, or fire protection systems and communications systems may be interrupted without prior approval of the Medical Center. Electrical work shall be accomplished with all affected circuits or equipment de-energized. When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without the Medical Center Director’s prior knowledge and written approval.
2. Contractor shall submit a request to interrupt any such services to the COR, in writing, a minimum of 15 calendar days in advance of proposed interruption. A minimum of 15 calendar days is required by Medical Center staff to assess interruption impacts and to prepare mitigation measures; therefore, the requirement to make utility interruption requests a minimum of 15 calendar days in advance of the planned interruption will not be waived. Request shall state specific details of the interruption to include identification of the utility system and device to be interrupted, its location, reason, date, exact time of, and approximate duration of such interruption.
3. Contractor will be advised (in writing) of approval of request, or of which other date and/or time that such interruption will cause least inconvenience to activities of Medical Center. Interruption time approved by Medical Center will occur at other than Contractor's normal working hours unless the VA determines that the interruption is of a minor nature and will not cause inconvenience to hospital activities.
4. If the Medical Center determines that the requested interruption is of a minor nature and will not cause inconvenience to hospital activities, the Medical Center may allow the interruption to proceed in less than 15 calendar days after receipt of the contractor’s request for interruption.
5. Contractor shall not manipulate any utility device that will interrupt any utility service.
Medical Center staff will operate utility devices to initiate and to terminate an approved utility service interruption. Contractor shall comply with lock out/tag out procedures.
6. In case of a contract construction emergency, service will be interrupted on approval of
Project Manager. Such approval will be confirmed in writing as soon as practical.
11.0 SAFETY PRECAUTIONS
1. The Contractor shall comply with all applicable Federal, State, and local legal requirements regarding workers health and safety. The requirements include but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR) Parts 1910 and 1926. Contractor is solely responsible for determining the legal requirements that apply to activities and shall ensure safe and healthful working conditions for its employees.
2. Contractor shall assume the responsibility to guard against causing of fires and/or explosions and to protect Government Property.
3. The Contractor shall perform the work in a manner consistent with the area security and fire safety regulations especially regarding exits and exit way access. Utility shutdowns shall not compromise security, communication, or fire safety for occupants.
4. No flammable liquids shall be stored or used in the medical center.
5. The necessary number and appropriate types of portable fire extinguishers are required per National Fire Protection Agency (NFPA) 10 and NFPA 241. Contractor shall keep certification on site at all times of extinguisher inspections.
6. The Contractor shall receive from the COR a permit for all cutting, welding, and soldering 24 hours in advance. All permits shall be prominently displayed during all construction.
7. All necessary precautions shall be taken by the contractor to prevent accidental operation of any existing smoke detectors or sprinkler heads.
8. The Contractor shall comply with an Infection Control Risk Assessment (ICRA) which will be developed with the COR and the Infection Control Practitioner assigned to the project.
Multiple ICRA's may be necessary to address specific risks at various stages of the project and must be approved prior to proceeding on each phase. The Contractor shall also comply with an Interim Life Safety Measures (ILSM) and Green Environmental Management System (GEMS) which will be developed with the COR.
12.0 HAZARDOUS MATERIAL REPORTING
1. The Contractor shall maintain hazardous material inventories and material safety data sheets (MSDS) for all hazardous materials (as defined in CFR 1910.120, 40 CFR's 355, 370, and 372) to be stored and used on this Medical Center. Hazardous materials must be inventoried when received and at the project's completion. The amounts used shall be maintained for the project duration, and for the calendar year (ending 31 December).
2. Hazardous Materials Inventories, Material Safety Data Sheets, and material quantities used shall be submitted to the COR for approval.
3. In the event of a spill, Contractor shall immediately notify the COR as well as the Contracting Officer. The Contractor shall be solely responsible for the expense of any cleanup of such spill, and the cleanup shall be in accordance with the applicable provisions of 40 CFR Part 761.
13.0 ENVIRONMENTAL PROTECTION
1. In order to provide for abatement and control of all environmentally hazardous materials arising from demolition and/or construction activities, the Contractor shall comply with all applicable environmentally hazardous material control and abatement and all applicable provisions of the Corps of Engineers’ Manual EM 385-1-1, “General Safety Requirements” as well as the specific requirements stated elsewhere in the Contract Documents.
2. Contractor is responsible for daily cleanup of all areas affected by construction. Construction areas in use or affected shall be returned to condition in which they were turned over or initially found. VA Housekeeping shall not be dispatched for cleaning associated with contractor construction.
3. Contractor shall take every precaution in preserving flooring, finishes, equipment, and furniture in areas of construction. Contractor shall repair or replace any damage incurred during construction at their expense.
4. Contractor shall use freight elevators for transmission of materials and personnel. Contractor shall take every precaution in preserving the elevators, including the hoist way and lobby doors, interior finishes, and shall conduct all good practices in observing lifting and motor components tolerances. Any damage incurred to any elevator component due to negligence will be repaired at expense of the contractor, within the workday of incurred damage.
14.0 Records Management
The following standard items relate to records generated in executing the contract:
1. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.
2. Contractor shall treat all deliverables under the contract as the property of the U.S.
Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.
3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records.
4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.
5. Contractor shall not create or maintain any records containing any Government
Agency records that are not specifically tied to or authorized by the contract.
6. The Government Agency owns the rights to all data/records produced as part of this contract.
7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.
8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.].
9. No disposition of documents will be allowed without the prior written consent of the
Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation.
Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.
10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (subcontractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any subcontractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
