STATEMENT OF WORK

Building 55 Accessibility Ramp

Project # 667-24-255

1. INTRODUCTION/BACKGROUND:

The Overton Brooks VA Medical Center located at 510 East Stoner Ave, Shreveport, LA 71101, has discovered the need to update and increase accessibility access to Building 55 (Specialty Clinic). There is currently a raised landscaping area that is located along the right side of the Main Entrance (when facing bldg. 55 from Veteran’s Way). This landscaping area is to be demolished and the ramp built in its place. This will allow direct access to the main entrance of Building 55 from the parking area that is directly south of the building.

a) Statement of Work: Provide Architect/Engineer and Construction professional services to include design development, construction documents, specifications, cost estimates, construction period services, construction site visits and construction. The scope of this project is to design and construct a new accessibility ramp that will provide easier access for veterans into Bldg. 55 (Specialty Clinic).

2. OVERVIEW OF WORK

a) Contractor must completely prepare site for building operations, including demolition, removal of existing landscaping structures, and the building of an accessibility ramp that complies with the VA Barrier Free Design Standard, for Project Number 667-24-255 at Overton Brooks VAMC Shreveport, Louisiana as required by specifications.

b) The Contractor must furnish all labor, provide all necessary materials, tools, equipment, superintendence, transportation, and must do all that which is necessary to the satisfactory and timely performance in accordance with this scope of work.

c) Quality Control (QC): The Contractor must submit a safety plan. The plan(s) must be approved by VA Safety Industrial Hygienist within 30 days of NTP.

d) The raised landscaping structure on Building 55 requires a total demolition, with the area being transformed into an accessibility ramp.

e) The Contractor to include in the proposal, demolition plans for the landscaping structure, and any utility removal. The VA will not provide any demolition design drawings or plans to contractor.

3. SCOPE OF WORK

a) DRAWINGS

This project will utilize AutoCAD, version 2020. All VA design guides/manuals and requirements can be found online at: https://www.cfm.va.gove/TIL/. Where conflicts occur between VA standards and applicable codes, the most stringent requirement shall govern. All drawings must be bundled using the “e-transmit” function.

b) Contractor to perform demolition of the raised landscaping area and properly dispose of all materials within the construction zone of Building 55. Contractor is to build an accessibility ramp that meets the VA Barrier Free Design Standard requirements for width, slope, and safety.

c) Contractor to preserve access to the Parking Lot to the south of the building (this area is part of a one-way U-Drive, this is the Exit of that U-Drive) existing sidewalk and wheelchair ramps (as much as possible), the existing Drop off lane that is directly in front of Building 55 (as much as possible) and preserve all parking signs.

d) The demolition, removal and disposal of the landscaping structure site must include but is not limited to the following:

1) Brick Form

2) Sprinklers, fixtures, and equipment

3) Rocks

4) Any Utilities (back to the meter): water, electrical, communication, sewer, and storm.

5) Anything not listed but found within the construction zone premises must be disposed of within the appropriate disposal methods.

Final clean-up of the area will be required.

4. GOVERNMENT FURNISHED INFORMATION

The following is a list of government furnished information found on the VA’s Technical Information Library website (https://www/cfm/va.gov/TIL/) to be used for this project, if applicable:

5. UTILITY SERVICES

a) This will include, but not limited to the following: building management system, telephone and data system, fire alarm system, physical security, and intercom system. Temporary means and after hour service must be included to ensure communication downtime is minimized.

b) All major utilities that are located where the ramp is to be built to be maintained and rerouted if necessary. Including: water, sewer, storm, electrical, communications, and fire suppression. Any water or electrical that were used in landscaping means must be demolished back to the main.

1) Responsibility of the contractor to coordinate utility shut down or removals with utility companies and VA COR. All utility outages require a 14-day written notice to the COR.

c) All utilities in the affected area must be located and flagged prior to any excavation work commencing.

6. PROJECT AREA LIMITATIONS

a) In Appendix A drawing shows the site layout, utilities, and locations of buildings within the site. The Project Area limitations must be everything inside the delineated area.

b) The contractor must store, stage, and contain all construction equipment within the site and laydown area.

c) Access to the site will be E. Stoner Ave to Veteran’s Way to Building 55, located to the southeast of the Front Circle. Any construction equipment damages caused to the road access points starting at E. Stoner Ave. to each point of Veteran’s Way and Building 55 will be repaired by the contractor.

7. NORMAL OPERATING HOURS:

Normal work hours are 7:30A.M. to 4:30P.M. (CT) Monday through Friday, excluding Federal Holidays and weekends. A maximum of 25% off hours are anticipated for this project. Contractors must obtain written permission from the Contracting Officer Representative (COR) at least 4 working days prior to work on Saturdays, Sundays, or Federal holidays.

8. SITE SUPERINTENDENT:

The Government must not exercise any supervision or control over the contractor’s employees performing services under this contract. Such employees must be accountable not to the Government, but solely to the contractor, who, in turn, is responsible to the Government. At all times during the performance of each task under this contract, and until the work under that task is completed and accepted, the contractor must directly oversee the work under each task, or assign, and have on work site, a competent representative who is satisfactory to the Contracting Officer and has authority to act for the contractor.

Contractor’s Employees. All work under this contract must be performed in a skillful and workmanlike manner. The Contractor may be required, in writing, to remove any employee deemed incompetent, careless, or otherwise objectionable by the Contracting Officer.

9. PROGRESS MEETINGS

Progress meetings must be held weekly between the contractor and the Government to discuss work progress, problems, and potential modifications.

10. PERIOD OF PERFORMANCE

This is a firm fixed price contract for full demolition, pulling back and capping off all utilities to the building, and finished landscaping.

A period of performance must be 180 calendar days.

11. REPORTING

a) All disposal waste materials to be provided to the VA COR monthly. Include category of waste, example Asbestos waste vs construction waste vs metal waste.

Provide the date of disposal with Cubic Yards of material disposed for each category of dumping involved in the project.

12. CONSTRUCTION SECURITY REQUIREMENTS

a) Contractor must be responsible for maintaining project security and safety from the public.

b) Contractor must maintain the fencing throughout the project period of performance.

13. CONSTRUCTION SIGNAGE

a) Contractor must follow all VA, Local, State and Federal regulations requirements for demolition project signage.

14. OPERATIONS AND STORAGE AREAS

a) Contractor lay down, storage, and staging area must be within the a fenced area.

b) Contractor must not park, store, or house any construction materials, equipment, or laborer vehicles in an area of that is not contained within the fenced zone.

c) Contractor must lock all storage, equipment, and staging equipment daily.

d) Contractor must lock the fence daily, only leave open for construction traffic.

Fence access to be always closed unless approved by the COR.

15. USE OF ROADWAYS

a) Access to the site will be E. Stoner Ave to Veteran’s Way to Building 55. All entry to the site will be through the gate access off E. Stoner Ave. Any construction traffic damages caused to the road access points will be repaired by the contractor.

16. PHOTOGRAPHY

a) Contractor must provide weekly progress photos of disposed materials during the period of performance.

b) Contractor must send weekly photos to VA COR within 3 days of capturing photos.

c) Images captured shall not contain patients, visitors, VA personnel, or contractor personnel.

d) Contractor must send final site photos to VA COR within 15 days of completed demolition and site clearing.

17. FINAL AS-BUILTS

a) Contractor to provide as-builts within 30 days of completed demolition and site clearing to VA COR to indicate final locations of remaining utilities, trees, and any other features of the site.

b) Contractor to provide to VA COR final tabulations in cubic yards of all disposal waste for each category involved in the project within 30 days of completed demolition and site clearing.
