About this file

This is a Quality Assurance and Surveillance Plan (QASP) for a Blanket Purchase Agreement (BPA) for Fire Protection and Life Safety (FPLS) Design Review and Statement of Conditions Services issued by the Veterans Health Administration (VHA), Healthcare Environment and Facilities Programs, under the Program Contracting Activity Central (PCAC). The document is dated February 20, 2026, and establishes minimum performance standards and quality metrics for third-party fire and life safety professional services delivered to VA Healthcare Systems across the United States and its territories.

The QASP establishes performance requirements for the Life Safety Agent (LsA) contractor across multiple service categories with defined Acceptable Quality Levels (AQLs). For professional conduct and general requirements, the LsA must maintain zero instances of contracting officer letters of concern, cure notices, or show cause letters; zero disciplinary actions from licensing authorities; and no more than one instance of late team member substitution notifications (beyond the required 15-day advance notice). For design review deliverables, the contractor must achieve 98% compliance rates on completeness of reviews covering all FPLS elements, accuracy of code references with proper edition dates, grammatical correctness and formatting, and on-time submission. For statement of conditions task orders, the LsA must achieve 98% compliance on site inspection completeness, drawing updates incorporating all FPLS elements per Joint Commission and VA requirements, building information document accuracy, code compliance, and deliverable quality. Additional requirements mandate 100% compliance with VHA infection control protocols during site visits and inspection procedures include 100% review by task order Contracting Officer's Representatives and random sampling by BPA Contracting Officer's Representatives. The plan details quality assurance procedures, defect responsibility determinations, and dispute resolution mechanisms across BPA and task order contracting structures.

3. QUALITY ASSURANCE SAMPLING PROCEDURES: Determinations of LsA performance will be sampled differently depending on the VA employee performing the quality assurance check.

a. BPA Contracting Officer (CO): The BPA CO will determine the frequency of any random or periodic quality sample inspections by BPA Contracting Officer’s Representative. The BPA CO will also work directly with the task order COs regarding any zero-defect quality metrics that are not being met that result in formal contract performance correspondence.

b. BPA Contracting Officer’s Representative (COR): The BPA COR will either request random samples of deliverables from the LsA (through the BPA CO) or from a task order COR. The report detailing adherence to, or deficiencies with the sample documents will be prepared by the BPA COR and shared with the BPA CO. If corrective actions are required, the BPA CO will direct the LsA to take such actions and the BPA COR will coordinate with the task order COR to minimize issues that may arise from the quality defect(s).

c. Task Order CO: The Task Order CO will work directly with the task order COR quality defects that require formal contract performance correspondence to document and/or correct.

d. Task Order COR: The Task Order COR will review 100% of all submittal deliverables from the LsA and determine their compliance with the SOW.

Routine submittal review comments for the LsA to address as required by the SOW are not considered quality defects unless unresolved by the final submittal.

If defects remain at the point of final submittal, the Task Order COR will prepare a report detailing the deficiencies with the deliverables and share it with the Task Order CO. The Task Order CO will direct the LsA to take corrective actions and will notify the BPA CO of the quality defect(s).

4. QUALITY DEFECT RESPONSIBILITY DETERMINATIONS: Determinations of quality defect responsibility will be the responsibility of the COs involved in reviewing defect reports. Both the LsA and the COR involved will outline their opinion on the cause of and which party(ies) were responsible for CO consideration. Specifically for each COR:

a. BPA COR: The nature of defects identified on deliverables either periodically or randomly inspected will generally be found after the Task Order COR has completed their work. Opinions on which party (LsA or Task Order COR) is responsible for the defect will depend on mitigating factors identified by the BPA COR as they investigate the root cause of the defect. In cases where the VA Task Order COR is determined to be solely responsible for the defect, the LsA may be requested to address and correct the defect to resolve a FPLS issue.

Such requests may result in a change order to the task order (if the contract is still active).

b. Task Order COR: Quality defects unresolved by the final submittal that in the opinion of Task Order COR are the responsibility of the LsA shall be noted in the report to the Task Order CO. The LsA will prepare their assessment of the defect and opinion on which party is responsible for the Task Order CO’s consideration. In cases where the VA Task Order COR is determined to be solely responsible for the defect, the LsA may be requested to address and correct the defect to resolve a FPLS issue. Such requests may result in a change order to the task order.

5. QUALITY DEFECT RESPONSIBILITY DISPUTES: LsA disputes over an assignment of defect responsibility shall be resolved in accordance with the clauses of the BPA contract.
