EXECUTIVE SUMMARY
The scope of this project has been updated to only account for egress out of Stair 2 (North stair tower) in Building 1. The Stair 2 discharge can be extended with an exit passageway through an existing Staff Room and exterior mechanical courtyard. The design team has detailed the pathway for the new exit in a concept drawing that includes new sidewalks leading to a public way.
An initial building walk-through with the VA was done to observe and discuss existing egress issues and possible solutions. The VA directive on site was to provide options to extend exit passageways for all stairways to an exterior public way. This would provide maximum flexibility for the facility during any future renovation project where an egress pathway may be temporarily unavailable. Since this initial walk thru, it has been determined that only the Stair 2 egress will be resolved.

PHYSICAL SECURITY
All new building elements and passageway additions are proposed within the building footprint and the interior courtyard. No blast-resistance design will be included.
All new exterior doors added to the building will always have free exit access. Controlled access can be provided from the exterior side of the new South Entryway door. Door operations and hardware will be developed in future submissions.
AE Works Ltd.
CONCEPT DEVELOPMENT
SCOPE OF WORK DESIGN GOALS
The main design goals for the project include the following:
▪ Provide an egress pathway improvement from Building 1, Stair Tower 2.
EXISTING FACILITY ASSESSMENT
BUILDING 1
▪ Stair 2 has 2 exits, South Corridor G027 and the radiology corridorat Staff Room
6A. There is an option to extend an exit passageway to an exterior discharge through the Staff Room and mechanical courtyard.
o It is impractical to construct a fire-rated passageway in Corridor G027 as this is a patient circulation pathway to Building 17 and is the main entrance to the Radiology suite.
o Code compliance upgrades will focus on a new exit passageway through the Staff Room and mechanical courtyard.
AE Works Ltd.
DESIGN OPTIONS
STAIR 2
Stair 2 will require construction of a new exit passageway to an exterior exit discharge.
The most practical option would involve a new pathway through the Staff Room and the mechanical courtyard.
The existing electrical closets may not be enclosed by the new passageway. Therefore, the pathway may not extend to the left of the stair door and must cross the existing corridor in a straight line.
The passageway must allow access from both sides to maintain egress from the
Radiology department. The doors are located within recesses to prevent the door swing from obstructing the egress pathway from the stairs. This configuration also has the smallest impact to the sitting area to the right of the stairs.
AE Works Ltd.
STAIR 2 EXIT PASSAGEWAY PLAN
A new exterior door would be installed at an existing window opening. The window opening must be enlarged to match the door size of Stair 2.
The remaining portion of the staff break room will remain a break room with an access door located outside the exit passageway.
The exterior portion of the passageway will require a security grille defining the egress pathway and separating the mechanical equipment from occupants. An opening is being provided in the existing CMU enclosure and a connecting sidewalk installed to complete the pathway.
AE Works Ltd.
STAIR 2 EXTERIOR EXIT PASSAGEWAY PLAN
The location of the new exterior door will avoid existing downspouts along the exterior wall.
The exterior pathway beyond the mechanical enclosure will require modified grading and new site lighting. A portion of the existing retaining wall and stairs will be removed.
The existing courtyard gate and the passageway between the enclosure and Building 1 is too narrow to meet egress requirements and is not considered to be used as part of the egress.
Any new mechanical equipment installed in the courtyard must be positioned to maintain the proposed egress pathway.

STAIR 2
Construction of a new exit passageway will require demolition of portions of the existing
Staff Room 6A. This includes removal of a portion of the corridor wall and room entrance.
A new exterior exit will require removal of an existing window and portion of the exterior wall. Thr exterior opening will have to be increased to match the size of the existing Stair 2 door.
The Stair 2 door will have to be removed and replaced so that it will swing in the direction of travel.
Exterior demolition work will include a new opening in the existing CMU mechanical enclosure and removal and / or repairs to the existing exterior retaining wall and stairs.
AE Works Ltd.
DESIGN
STAIR 2
The new interior exit passageway walls are required to be fire barriers. They will be constructed out of 6” CMU. A horizontal fire-rated barrier will be built between the existing ceiling plane and the passageway and will have a finished gypsum board surface. All rated assemblies must be tested in accordance with ANSI/UL 263 or
ASTM E119 as noted is specifications.
All new interior doors and frames will be hollow metal and are required to have a 90-minute fire resistance rating if located in new rated passageway. Vision panels are permitted in 90-minute doors at the maximum size tested. All doors will have panic hardware in the direction of egress travel.
The new exterior door will have panic hardware in the direction of egress travel.
A new security grille barrier to define the egress pathway is required within the mechanical courtyard.
FINISH MATERIALS
Interior finish materials will be selected as a basis of design to complement the existing medical center for cohesive design aesthetics and standard material maintenance by staff.
Where possible, finishes will match adjacent spaces that have been newly renovated to keep a consistent aesthetic to blend existing with new. Where necessary, some flooring materials will be suggested to be completely new in corridors where older VCT flooring cannot be matched. Materials selected are intended to be durable, sustainable and follow existing maintenance protocols.
Colors and patterns will be presented as part of the complete interior aesthetic package in follow up submissions. This palette of materials will be selected to promote a healing interior environment. Material selections will follow the requirements listed in the VA
Program Guide PG-18-14, Room Finishes, Door, and Hardware Schedule.

Limited Asbestos Building Inspection Lebanon, Pennsylvania
October 13, 2022
EXECUTIVE SUMMARY
On September 27, 2022, Pennoni was contracted by AE Works to perform a pre-renovation limited asbestos inspection at the facility located at 1700 South Lincoln Avenue, Lebanon, Pennsylvania 17042. This limited inspection was performed to identify Asbestos Containing Materials (ACM) present within the facility in areas identified to be impacted by renovations.
Suspect ACM Bulk samples (floor tile, wall systems, ceiling tile, and other building components) were collected and analyzed. The suspect materials sampled found to contain greater than one percent (>1%) asbestos were tan 9” x 9” floor tile, brown 9” x 9” floor tile, and dark brown 12” x 12” floor tile in the hallway outside Stairwell 2. Suspect material not sampled, but presumed to be ACM included the fire door entering into Room 6A. Recommendations are found at the conclusion of this report.
1. LIMITED ASBESTOS CONTAINING MATERIAL ASSESSMENT
On October 4, 2022, Pennoni personnel (Michael Potts [PA DOLI Certified Asbestos Inspector, Certification #061136]) performed a visual and tactile review of the suspected ACM within the facility in areas identified to be impacted by renovations and sampled said potentially impacted suspect ACM. The samples were submitted to EMLab P&K and analyzed using Polarized Light Microscopy (PLM). EMLab P&K is a National Voluntary Laboratory Accreditation Program (NVLAP Lab Code 200844-0) approved laboratory. Results are compared to the Occupational Safety and Health Administration (OSHA)/U.S. Environmental Protection Agency (U.S. EPA) definitions of asbestos (>1%).
Laboratory analytical results summarized in Table 1 below can be found in Appendix A.

2. CONCLUSIONS
• The tan 9” x 9” floor tile, brown 9” x 9” floor tile, and dark brown 12” x 12” floor tile in the hallway outside Stairwell 2 were found to meet the EPA definition of asbestos containing material (greater than one percent [>1%] asbestos). The fire door entering into Room 6A was not sampled but presumed to contain asbestos. The flooring materials are Category I non-friable and can be disposed of as construction and demolition (C&D) waste. However, if material is rendered friable during abatement activities, it must be treated as Regulated Asbestos Containing Material (RACM) and abated in accordance with applicable federal (U.S. EPA, OSHA, Department of Transportation [DOT]), Pennsylvania (Pennsylvania Department of Environmental Protection [PA DEP]/Pennsylvania Department of Labor and Industry [DOLI]) and local regulations governing the removal, transport, and disposal of asbestos. The fire door is RACM and must be treated as outlined above.
• All other suspect ACM Bulk samples collected and analyzed were found to be non-detect for asbestos and therefore do not meet the EPA definition of asbestos containing material (greater than one percent [1%] asbestos).
3. RECOMMENDATIONS
• Asbestos abatement activities are recommended prior to any repair, renovation or demolition activities which would disturb the matrix of confirmed or Presumed Asbestos ContainingMaterials.
It is recommended that asbestos activity be performed in accordance with applicable U.S. EPA, U.S.
Department of Labor, OSHA, PA DEP, PA DOLI regulatory requirements. In order to comply with these requirements, the following is recommended:
1. Written notification to PA DEP and PA DOLI of demolition/renovation ten (10) working days prior to asbestos related activities, as required by the National Emission Standards for Hazardous Air Pollutants (NESHAPS) of the Clean Air Act, if material is or is expected to become friable.
2. Development of a written Asbestos Abatement Project Design Specification, dictating abatement work practices and control measures to reduce airborne fiber concentrations, waste disposal requirements and air sample collection and analysis requirements including the final clearance (re-occupancy) criteria.
3. Selection of a PA DOLI licensed Asbestos Contractor for the performance of asbestos response activities.
4. Air sample collection and analysis subsequent to asbestos abatement activities in compliance with project design requirements for the re-occupancy by non-protected individuals.
Occupant notifications concerning asbestos abatement are required ten (10) working days prior to their disturbance.
Limited Asbestos Building Inspection
• The site is required to maintain this report indefinitely in accordance with the OSHA Medical Recordkeeping Requirements identified in 29 CFR 1910.1020 and inform employees annually of their rights of access. If such a plan has been developed, an update based on the findings of this report to the site Operations, Maintenance and Repair Plan is recommended.
A copy of existing asbestos inspection reports must be provided to each contractor involved in the project.
4. CERTIFICATION
The information contained in this report is believed to be accurate and true to the best knowledge of the inspector(s). Findings and recommendations for this investigation are based on the observations of the conditions, as they existed at that time.
