About this file

This document is Addendum 1 for Oregon Department of Transportation (ODOT) Request for Proposal (RFP) #S-73000-00013619 for Mobile Audiometric Services. The addendum modifies the original RFP to expand audiometric testing services across ODOT Regions 1, 2, 3, 4, and 5, with the contractor expected to conduct approximately 1,000 audiograms per year for ODOT personnel. Testing sites will include multiple locations across Oregon such as Clackamas, Cascade Locks, Sandy, Astoria, Salem, Eugene, Ashland, Bend, Baker City, and others. The contractor will provide mobile audiometric testing and counseling services, including testing at various regional sites, potentially covering day and night shift crews, and will work in coordination with ODOT Safety Managers who are CAOCH certified technicians.

The addendum also revises the evaluation process, specifically the Equipment Capability and Quality section, which now carries a maximum of 500 points. Proposers must provide detailed information about their testing equipment, including age, quality, capability, accuracy, calibration processes, and certification. While the original RFP indicates a not-to-exceed amount of $150,000 and is contingent on Oregon Legislative funding, this addendum focuses on clarifying service requirements and evaluation criteria. The procurement includes an Aspirational Target of 12% for Certified Small Businesses and allows for potential additional points for Oregon-based services and use of recycled materials. Protests to the addendum must be submitted by 05/06/2025 at 5:00 p.m. Pacific Time.
