About this file

This document is an Attachment E - Price Proposal Form for a Structural Prep Kit Equipment Maintenance contract opportunity issued by the State of Oregon, acting through the Department of Oregon State Police on behalf of the Oregon Office of the State Fire Marshal. The contract is for a five-year term and covers maintenance services for structural prep kit equipment. Interested offerors must submit pricing for the various services described, which may include a percentage off discount to an established published price list. The evaluation process is designed to award the contract to the offeror whose proposal best meets the requirements, not necessarily the lowest cost. Pricing must be held firm for 30 days.

The pricing table in the attachment allows offerors to provide per-unit pricing for the various services required. Offerors may utilize the sample table provided or create their own pricing table. All costs associated with providing the required services must be incorporated into the offeror's proposed pricing.
