About this file

This is a sample personal/professional services contract template between the State of Oregon, acting through its Oregon Department of Human Services (ODHS), Office of Vocational Rehabilitation Services, and a contractor to serve as a Ticket-to-Work (TTW) partner. The contract supports the Social Security Administration's Ticket-to-Work program by providing employment services to SSA beneficiaries aged 18-65 receiving Social Security Disability Insurance (SSDI) and/or Supplemental Security Income (SSI). The TTW Partner's primary responsibilities include identifying staff liaisons for ticket coordination, submitting beneficiary referral documentation to VR for claims processing, conducting at least quarterly contact with ticketholders to assess service needs, maintaining contact documentation, implementing training on TTW processes, and collaborating with ODHS on beneficiary education and benefits counseling. ODHS will oversee IPE (Individual Plan for Employment) maintenance, provide training on comprehensive TTW program procedures, operate as the administrative unit for ticket tracking and claims processing, and provide annual reports to TTW partners listing tracked SSA beneficiaries and milestone-outcome payments. The contract duration is not specified in the template, though amendments allow for extensions up to five years total with potential cost-of-living adjustments.

The contract structure uses a milestone and outcome payment model wherein VR retains 50% of Phase 1 payments and the TTW Partner receives 50%, while Phase 2 and Outcome payments are split 20% to VR and 80% to the TTW Partner. Payments will be made within 45 calendar days of SSA payment receipt, and the TTW revenue is excluded from other ODHS job placement contract reporting requirements. ODHS will not reimburse travel or additional expenses. The sample contract does not specify a monetary not-to-exceed amount, as this is a template document. Federal funding source is identified as Assistance Listing 84.126. The contractor must maintain commercial general liability insurance of at least $1,000,000 per occurrence and $2,000,000 annual aggregate, comply with workers' compensation requirements, and obtain additional insured endorsements naming the State of Oregon. Contractor must also comply with extensive federal and state requirements including civil rights laws, audit requirements under 2 CFR Part 200, debarment and suspension regulations, confidentiality provisions for SSA and Federal Tax Information, background check requirements, abuse reporting mandates, and nondiscrimination provisions regarding race, religion, national origin, sex, age, and disability.
