About this file

This is an Applicant Information and Certification Sheet (Attachment A) for a Request for Applications issued by the Oregon Department of Human Services (ODHS) Vocational Rehabilitation (VR) program for participation in the Ticket to Work (TTW) Program, which implements the Social Security Administration's Ticket to Work and Self Sufficiency Program for SSA beneficiaries as described in 20 CFR Part 411. The document serves as the certification form that applicants must complete and sign to submit a responsive application. By submitting the completed form, applicants certify their understanding and acceptance of all RFA requirements, acknowledge receipt of any addenda, and agree that their application constitutes a firm offer valid for 180 days following the closing date. Applicants commit to performing the scope of work and meeting performance standards as outlined in the final negotiated contract scope of work.

The certification sheet requires applicants to attest to numerous compliance and eligibility requirements, including tax law compliance, non-discrimination in employment and subcontracting practices, implementation of sexual harassment and discrimination prevention policies (for contracts anticipated at $150,000 or more), wage equity compliance under ORS 652.220, compliance with broadband internet access service regulations if applicable, and verification that neither the applicant nor its employees, agents, or subcontractors appear on the Office of Foreign Assets Control's Specially Designated Nationals and Blocked Persons list or the System for Award Management exclusions lists. Applicants must also certify the absence of conflicts of interest, confirm the truthfulness and accuracy of all application contents, and acknowledge that false statements or misrepresentations could constitute false claims under the Oregon False Claims Act with associated liabilities and penalties.
