About this file

This document comprises the required federal forms for the Rhode Island Department of Transportation (RIDOT) that must be completed and submitted with bid offers for federally-funded contracts. The forms establish compliance requirements for contractors bidding on RIDOT projects, including the shuttle bus services contract (Project Number 2026-CB-019, Corridor-Ashaway) for Rhode Island College. Bidders must submit an anti-collusion certificate declaring they have not engaged in any agreements or collusion to restrain competitive bidding, a certification regarding debarment and suspension status, a conflicts disclosure statement identifying any family or personal relationships with RIDOT employees, and a disclosure of lobbying activities form. Proposal submissions are due August 7, 2026 at 11:00 AM EDT, with vendor questions due by July 22, 2026 at 2:00 PM EDT. The contract period runs from August 31, 2026 through August 30, 2029, with options to renew for up to two additional 12-month periods.

The forms require contractors to certify they are not presently debarred, suspended, or ineligible from federal transactions and have not been convicted of fraud, criminal offenses, or violations of antitrust statutes within the three-year period preceding the proposal. Contractors must also commit to compliance with all non-discrimination statutes and authorities, including Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Americans with Disabilities Act, and the Age Discrimination Act of 1975. The document includes USDOT Standard Title VI/Nondiscrimination Assurances requiring contractors to ensure non-discrimination in the selection of subcontractors, procurement of materials, and employment practices, with sanctions for noncompliance including withholding payments or contract cancellation. All forms must be signed by a duly authorized representative and submitted electronically through Ocean State Procures as part of the bidding process.
