Bidders may expand each of the response (white) spaces within this document in order to provide a full response to each requirement.

A. Evaluation of Maine’s Rural Health Transformation Program

1. Identify all required data sources needed to construct measures, (Program Year 1 = “baseline” measures; Program Years 2-5 = “ongoing” measures), for the four (4) overarching performance objectives and metrics corresponding to the five (5) major initiatives (refer to the State’s initial RHTP application project narrative, pages 9 and 52) outlined in the State’s final Rural Health Transformation Program (RHTP) plan, including, but not limited to:

a. Establishing contact with, and requesting data from, various sub-awardees, vendors, partners, and subcontractors, including the Maine Health Data Organization, other State offices/departments, HealthInfoNet, and other relevant organizations as identified by the Department.

i. For any metrics or performance objective measures without an existing data source, identify and work with the relevant party to obtain data for developing metrics or performance objective measures.

b. Complying with any Department and/or U.S. Centers for Medicare and Medicaid Services (CMS) changes/revisions to metrics and performance objective measures submitted as part of the State’s initial RHTP application.

c. Receiving and cleaning data from multiple sources in various file formats, such as claims data, electronic survey responses, training logs, and other written reports.

2. Using the identified and approved data sources, construct metrics and performance objective measures.

a. Use data collected annually (and historical data (when appropriate)) to construct metrics and performance objective measures.

i. Where feasible and as outlined in the State’s RHTP plan, construct both Statewide metrics and performance objective measures alongside granular geographic metrics and measures, such as county-level measures.

b. Design and implement a methodology(ies) for constructing metrics and performance objective measures to ensure consistency and replicability across all five (5) program years.

c. Provide a detailed explanation of the methodology(ies) used for data collection and metric and performance objective measure construction, including any data limitations and interpretability considerations.

d. Apply the developed methodology(ies) for collecting required data for metrics and performance objective measures on a cadence as directed by the Department.

3. Receive data and track metrics and performance objective measures throughout the program duration.

a. Establish a consistent method and schedule for collecting data regularly from various data sources.

b. Develop a schedule for constructing metrics and performance objective measures in relation to timing of receipt of required data sources.

c. Track and assess if metrics and performance objective measures are approaching benchmarks and program goals, as outlined in the State’s final RHTP plan.

i. During Program Years 2-5, compare current program year metrics and performance objective measures to the previous year’s metrics and measures (i.e., year 2 compared to year 1, year 3 compared to year 1 and 2, etc.).

B. Communication and Reporting of Metrics and Performance Objective Measures

1. On an annual basis, present metrics and performance objective measures in a clear and transparent manner.

a. Track and assess if metrics and performance objective measures are approaching benchmarks and program goals, as outlined in the State’s final RHTP plan.

b. During Program Years 2-5, compare current year metrics and performance objective measures to all prior years’ metrics and measures.

2. Provide quarterly progress reports and annual program reports that synthesize, analyze, and interpret all metrics and performance objective measures.

3. Disseminate and communicate findings to the State’s RHTP Governance Committee, and others as requested by the Department.

a. During Program Years 2-5, discuss the State’s RHTP progress based on trends in metric and performance objective measures across program years.

i. Create and share with the Department no later than December 1, 2026, a public-facing data dashboard to accompany the metrics and performance objective measures report, which:

1) Enables user-friendly exploration of the metrics and performance objective measures, including analyses at various geographic levels (where applicable), using a platform such as Tableau or PowerBI.

2) Incorporates regular updates and changes in metrics and performance objective measures over time.

3) Includes a component that maps program impact across different areas of the State where geographic stratifications are available for each metric and performance objective measure.

b. Share updates to the dashboard with the Department, annually, by December 1, including collection of Performance Years 2-5 ongoing metrics and performance objective measures.

4. Communicate progress in obtaining required data sources, developing metrics and performance objective measures, and preparing the annual report and accompanying dashboard.

a. Meet with the Department on a weekly basis (or other schedule as agreed upon between the Department and awarded Bidder) to discuss progress.

b. Communicate challenges in obtaining and/or processing data or in conducting analyses to develop metrics and performance objective measures to the Department in a timely manner.

C. Ad-Hoc Work
