C.1 Statement of Work

DESCRIPTION/SPECIFICATION/STATEMENT OF WORK

4A3W Repair Underground Steam Line

I. Background The Federal Correctional Institution (FCI) Milan, MI requires the repair of an existing underground 10” Perma-Pipe steam piping sys-tem. During an Infrared Leak Detection Survey completed on February 13, 2013, there were three (3) areas of concern. The inspec-tion conducted by a Perma-Pipe representative has been completed and will be provided to aid in the general location of the leak(s).

The contract will be for the labor and materials needed to excavate the areas of concern, repair the steam pipe, insulate around the areas of repair, backfill per manufacturers recommendations, asphalt repair, and landscape restoration.

II. Place of Performance Performance of the required underground steam pipe repairs will take place outside the secure perimeter of the Federal Correctional Institution located at 4004 E. Arkona Rd, Milan, MI. This facility is located in the county of Washtenaw and at the intersection of San-ford and Arkona Rd. All work associated with this project will be performed at the facility and no offsite workplace is expected.

III. Schedule This selected contractor will be given a notice to proceed no later than June 03, 2013. The selected contractor has 10 days from being given a notice to commence work, and must be 100% complete with all aspects of the contract within 70 working days from the start of work. As outlined below the steam piping that must be repaired is essential to the orderly operation of a correctional institution.

Careful consideration of normalized seasonal temperatures by both the contractor and FCI Milan as to the when the repairs commence will be reviewed during this pre-construction meeting.

IV. Systems/Equipment to be repaired

1. Earthwork The qualified contractor will provide the equipment necessary to conduct all aspects of the project to in-clude the following.

1.1. Excavation: Large scale excavation equipment with the capability to remove soil up to 15’ below grade.

1.2. Dewatering: Localized dewatering equipment may be necessary to remove groundwater in and around the areas of repair.

1.3. Trench Protection: Protection of the workers will be accomplished through properly sloping of the trench banks, trench boxes and/or shoring.

1.4. Provide temporary roads as necessary. The leak appears to be located under an existing asphalt parking lot, near the gravel perimeter security road, and under the main institution truck access road.

1.4.1. Once excavation begins the contractor must provide ample lighting during the hours between dusk and dawn to ensure the en-tire excavation area is visible to the Perimeter Patrol and/or any other vehicles

1.4.2. At all times during the work, the Perimeter Patrol vehicles will be given a driving path parallel and in direct line of sight of the entire Perimeter Security Fence. It may be necessary to provide a temporary road with signage, cones, barricades, and/or berms to en-sure the Perimeter Patrol vehicles can maintain the security of the institution.

1.4.3. One of the main institution roads will be affected by the work; access to vehicles and semi-trucks must be accommodated at all times. It may be necessary to provide a temporary road around the excavation site.

1.4.4. Access to the north Powerhouse drive must be maintained, as it will remain in operation during the work.

1.5. Roadway, Parking Lot, and Grade Restoration: At the completion of the work all roadways and parking lots must be re-stored to their pre-construction conditions.

1.5.1. The Powerhouse parking lot does contain a drainage system below the asphalt; this too will have to be brought back to precon-struction conditions.

1.5.2. All grass areas and ditches will be restored to a finished condition, to include grass seeding.

1.6. There are existing underground utilities potentially in the vicinity of where the work will take place. The contractor will utilize the Miss Dig System or 811 location services. FCI Milan staff will provide additional location of utilities in the affected areas

RFQP04171300004 Page 6 of 32 as well. Care must be given to ensure these utilities are not damaged during any part of the excavation.

1.6.1. There is a 6” high pressure natural gas line running parallel to the road on the east side.

1.6.2. There is an 8” domestic water line running parallel to the road on the east side.

1.6.3. Running parallel to the 10” steam line that must be repaired is a 4” Perma-Pipe condensate pipe, 4” Hot Water supply pipe (Not in Use), and a 1 ½” Hot Water Recirculation pipe (Not in use).

1.6.4. There is a communication duct bank running parallel to the road on the west side.

1.6.5. There is an institution telephone line that bisects the pipe under the west edge of the truck access road.

1.6.6. Near the Secure perimeter fence there are several institution owned electric and communication conduits that are currently in use.

1. Steam Piping Repair: The repair will consist of locating all leaks, repair of the affected areas, weld in new piping if necessary, non-destructive testing of all welds, and complete restoration of the Perma-Pipe system.

2.1. The contractor will be qualified by Perma-Pipe for the installation and repair of their products and provide a certified welder(s) capable welding schedule 80, 10” 150 PSI Steam and with the ability to meet the coordination criteria set forth in the next section.

Certification of the welders will be ASME or equivalent.

2.2. FCI Milan will provide an inspection report conducted by a Perma-Pipe representative that delineates the location of areas be-lieved to be leaking.

2.3. FCI Milan will retain the services of a Perma-Pipe representative for one full day prior to the commencement of work to assist the contractor in the development of the best means and methods necessary to accomplish the work.

2.4. FCI Milan will provide a total three days of Perma-Pipe representative at the conclusion of the pipe repairs to witness all non-destructive testing, verify proper restoration of the Perma-Pipe system, and to certify conformance of the work. This must be accom-plished prior to any work being covered and/or buried.

2.5. Contractor will provide all materials necessary to effectuate all repairs.

1. Coordination: Coordination of all activities involved in this project is by far the most important aspect of this work.

3.1. The Steam carried in the piping that needs to be repaired is the sole source of heat for the institution housing over 1500 in-mates. It also provides the domestic hot water necessary for personal hygiene and institutional sanitation, meal preparation, and laun-dry services. Every effort possible must be made to minimize steam shut downs. Additionally, all repairs must be conducted in the most expeditious manner.

3.2. Steam service to the institution cannot be interrupted for more than a twelve hour duration. There must be at least 24 hours of notice given prior to the interruption.

3.3. The contractor is responsible to chain, lock-out, and tag-out a minimum of two valves between the boilers and the work within the Powerhouse and one valve on the downstream side of the work in the Facilities Annex located within the secure perimeter.

3.4. An onsite meeting will be held prior to any repairs being conducted. The FCI Milan Project Representative will be the point of contact and alert all FCI Milan personnel of the meetings. The contractor will provide all possible scenarios involved with how the re-pairs will be made.

3.5. All questions and/or clarifications by the contractor will be in writing and provided to the Governments Representative.

1. Reporting: At the conclusion of each repair the contractor will provide FCI Milan with a copy of the third party non-destructive testing results. Testing results will definitively note pass or fail.

1. 5. Noted Deficiencies: In the event the contractor identifies any unforeseen site condition or any item that is over and above this scope of work it must be immediately brought to the attention of the Government’s representat-ive. The Contracting Officer is the only person authorized to obligate additional funding for this project. Additionally, any and all modifications made to this scope of work must be reviewed and approved by the Regional Office located in Kansas City Kansas.

V. Description of Responsibilities The successful contractor will submit a standalone proposal based upon the information provided in this document, provided draw-ings, and provided Perma-Pipe inspection to repair the underground 10” Perma-Pipe steam pipe system. These responsibilities will include the requirement of all personnel onsite for more than 4 days in a calendar year to complete a background investigation, finger printing, drug testing, residency requirement, and a mandatory 4 hour institution familiarization class as well as any other security re-quirements under the Presidential Directive of HSPD-12. Contractors will be responsible for inventorying their tools upon entry to the facility and exiting to ensure that all tools are accounted for before leaving.

DJAR-PGD-05-08 Implementation of Homeland Security Presidential Directive (HSPD) 12 - Policy for a Common Identification Standard for Federal Employees and Contractor

NOTICE OF CONTRACTOR PERSONNEL SECURITY REQUIREMENTS

Compliance with Homeland Security Presidential Directive-12 (HSPD-12) and Federal Information Processing Standard Publication 201 (FIPS 201)' entitled "Personal Identification Verification (PIV) for Federal Employees and Contractors," Phase I.

1. Long-Term Contractor Personnel:

In order to be compliant with HSPD-12/PIV I, the following investigative requirements must be met for each new long-term2 con-tractor employee whose background investigation (BI) process begins on or after October 27, 2005:

a. Contractor Personnel must present two forms of identification in original form prior to badge issuance (acceptable documents are listed in Form 1-9, OMB No. 1615-0047, " Employment Eligibility Verification," and at least one document must be a valid State or Federal government-issued picture ID);

b. Contractor Personnel must appear in person at least once before a DOJ official who is responsible for checking the identification documents. This identity proofing must be completed sometime during the clearance process but prior to badge issuance and must be documented by the DOJ official;

c. Contractor Personnel must undergo a BI commensurate with the designated risk level associated with the duties of each position.

Outlined below are the minimum BI requirements for each risk level:

•High Risk - Background Investigation (5 year scope) •Moderate Risk - Limited Background Investigation (LBI) or Minimum Background Investigation (MBI) •Low Risk - National Agency Check with Inquiries (NACI) investigation

d. The pre-appointment BI waiver requirements for all position sensitivity levels are a:

1) Favorable review of the security questionnaire form;

2) Favorable fingerprint results;

3) Favorable credit report, if required;3

4) Waiver request memorandum, including both the Office of Personnel Management schedule date and position sensitivity/risk level; and 5) Favorable review of the National Agency Check (NAC)4 portion of the applicable BI that is determined by position sensitivity/risk level.

A badge may be issued following approval of the above waiver requirements.

If the NAC is not received within five days of OPM' s scheduling date, the badge can be issued based on a favorable review of the Se-curity Questionnaire and the Federal Bureau of Investigation Criminal History Check (i.e., fingerprint check results).

e. Badge re-validation will occur once the investigation is completed and favorably adjudicated. If the BI results so justify, badges is-sued under these procedures will be suspended or revoked.

2. Short-Term Contractor Personnel:

It is the policy of the DOJ that short-term contractors having access to DOJ information systems and/or DOJ facilities or space for six months or fewer are subject to the identity proofing requirements listed in items la. and lb. above. The pre-appointment waiver re-quirements for short-term contractors are:

a.Favorable review of the security questionnaire form;

b.Favorable fingerprint results;

c.Favorable credit report, if required;5 and d.Waiver request memorandum indicating both the position sensitivity/risk level and the duration of the appointment. The commen-surate BI does not need to be initiated.

A badge may be issued following approval of the above waiver requirements and the badge will expire six months from the date of is-suance. This process can only be used once for a short-term contractor in a twelve month period. This will ensure that any consecutive short-term appointments are subject to the full PFV-I identity proofing process.

For example, if a contractor employee requires daily access for a three or four-week period, this contractor would be cleared according to the above short-term requirements. However, if a second request is submitted for the same contractor employee within a twelve-

RFQP04171300004 Page 9 of 32 month period for the purpose of extending the initial contract or for employment under a totally different contract for another three or four-week period, this contractor would now be considered "long-term" and must be cleared according to the long-term requirements as stated in this interim policy.

3. Intermittent Contractors:

An exception to the above-mentioned short-term requirements would be intermittent contractors.

a. For purposes of this policy, "intermittent" is defined as those contractor employees needing access to DOJ information systems and/ or DOJ facilities or space for a maximum of one day per week, regardless of the duration of the required intermittent access. For ex-ample, the water delivery contractor that delivers water one time each week and is working on a one-year contract.

b. Contractors requiring intermittent access should follow the Department's escort policy. Please reference the August 11, 2004, and January 29, 2001, Department Security Officer policy memoranda that conveys the requirements for contractor facility escorted ac-cess.

c. Due to extenuating circumstances, if a component requests unescorted access or DOJ IT system access for an intermittent contract-or, the same pre-employment background investigation waiver requirements that apply to short-term contractors are required.

d. If an intermittent contractor is approved for unescorted access, the contractor will only be issued a daily badge. The daily badge will be issued upon entrance into a DOJ facility or space and must be returned upon exiting the same facility or space.

e. If an intermittent contractor is approved for unescorted access, the approval will not exceed one year. If the intermittent contractor requires unescorted access beyond one year, the contractor will need to be re-approved each year.

4. An individual transferring from another department or agency shall not be re-adjudicated provided the individual has a current (within the last five years), favorably adjudicated BI meeting HSPD-12 and DOJ's BI requirements.

5.The DOJ's current escorted contractor policy remains unchanged by this acquisition notice.

NOTES:

1. FIPS 201 is available at: www.csrc.nist.gov/publications/fips/fips201 /FIPS-201-22505.pdf.

2. Under HSPD-12, long-term contractors are contractors having access to DOJ information systems and/or DOJ facilities or space for six months or longer. The PIV-I identity proofing process, including initiation and adjudication of the required background investiga-tion, is required for all new long-term contractors regardless of whether it is the current practice to issue a badge. The second phase of HSPD-12 implementation (PIV-II) requires badge issuance to all affected long-term contractors.

3. For contractors in position sensitivity/risk levels above level 1, a favorable review of a credit check is required as part of the pre-appointment waiver package.

4. In order to avoid a delay in the hiring process, components should request an Advance NAC Report when initiating investigations to OPM. Per OPM 's instructions, to obtain an Advance NAC Report, a Code " 3" must be placed in block "B " of the " Agency Use Only " section of the investigative form. This report is available for all case types.

5. For contractors in position sensitivity/risk levels above level 1, a favorable review of a credit check is required as part of the pre-appointment waiver package.
