About this file
This Request for Quotation (RFQ) document is issued by the West Virginia Department of Shared Administration Purchasing Division on behalf of the Department of Health Facilities, John Manchin Sr. Health Care Center, to solicit bids for Registered Dietician Services. The solicitation seeks to establish a contract for a Registered Dietician to provide comprehensive dietary services at the facility, which serves a patient population ranging from age 18 to 100+. The scope of work includes writing approved plans of correction, implementing corrective actions, developing care plans, creating menus for long-term care facilities, and writing specialized diets for patients with chronic and terminal illnesses. The vendor must provide services during normal business hours (8:00 AM to 4:00 PM, Monday through Friday) and work a maximum of 40 hours per month, with additional hours requiring prior administrative approval. The document does not specify explicit response deadlines, due dates, site visits, bidder conferences, award dates, or contract renewal options.
The contract will be awarded to the vendor offering the lowest overall total cost that meets all specified requirements. Vendors must submit pricing on a per-hour basis with a total price for hours requested via wvOASIS or as Exhibit A, with incomplete pricing pages resulting in bid disqualification. Vendors are responsible for all travel costs, mileage, and travel time, which may be incorporated into the hourly rate but will not be reimbursed separately by the agency. Minimum qualifications include a valid West Virginia Dietician's license, a minimum of ten years of experience, and knowledge of Federal and West Virginia regulations and survey processes. Vendors must designate a primary contract manager available during business hours and comply with the facility's security protocols. The document establishes vendor default provisions and remedies, including immediate contract cancellation for non-compliance with specifications, requirements, or applicable laws.
