About this file

This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for the procurement of a Digital Folding Wheelchair Scale for the Illinois Veterans' Home - LaSalle. The RFQ outlines the required specifications for the scale, including a 1000 lb capacity, dual ramp, and a 32" x 37" platform. The requested quantity is 1 unit, and the vendor must provide the brand name and model number in their quote. The RFQ stipulates that all quotes must include shipping and handling charges, and the awarded vendor will be chosen based on the lowest total bid. The vendor is required to contact the Department of Labor to ensure understanding of prevailing wage requirements, and the quoted prices must be valid for 180 days from the date of submission.

The RFQ also includes terms and conditions, such as the right of the agency to reject all offers or individual offers, award items individually or in total, and waive minor defects. The agency reserves the right to partially ship equipment in case of a delay on the total order. The use of a brand name is for the purpose of describing the standard of quality, performance, and characteristics desired and is not intended to limit or restrict competition. "Or equal" submissions will not be rejected due to minor differences in design, construction, or features that do not affect the suitability of the product for its intended use.
