About this file
This is a Proposer Information and Certification Sheet (Attachment A) for RFQ S-12300-00015608 issued by the Oregon Department of Administrative Services Building and Development (OBDD) for Percent for Art Program Coordinator Services. The document serves as a standardized certification form that prospective proposers must complete and submit as part of their proposal response. The RFQ requires proposers to provide their legal entity information, contact details, state of incorporation, and Oregon Business Registry Number if applicable. Proposers must certify their understanding and acceptance of all RFQ requirements and agree to be bound by contract terms outlined in Attachment F, as well as acknowledge receipt of any addenda. The proposal constitutes a firm offer valid for 180 days following the closing date, and successful proposers agree to perform the specified scope of work and meet performance standards in the final negotiated contract.
The certification sheet establishes mandatory compliance requirements covering tax compliance with Oregon Department of Revenue, non-discrimination in employment and subcontracting practices, prevention of sexual harassment and discrimination (required for contracts valued at $150,000 or more), equal wage compensation, employee communication rights regarding compensation, and broadband internet service compliance. Proposers must also certify that neither they nor their employees, agents, or subcontractors appear on federal exclusion lists maintained by the Office of Foreign Assets Control or the System for Award Management. Additionally, proposers must declare the absence of conflicts of interest with the State and certify that all proposal contents are truthful, accurate, and prepared independently without collusion or fraud. Proposers acknowledge that false statements or misrepresentations could result in liability under the Oregon False Claims Act.
