About this file
This document is Exhibit E of Request for Quotes (RFQ) 260563 issued by the Orange County Transportation Authority (OCTA), containing Level 2 Health, Safety and Environmental (HSE) Specifications applicable to the window covering supply and maintenance contract. The specifications establish comprehensive health, safety, and environmental compliance requirements that the winning contractor and all subcontractors must follow throughout the three-year contract term. Key requirements include designation of a Qualified Individual as the Health and Safety Representative, mandatory hazard evaluation and documentation prior to performing any work activities, immediate notification to OCTA's designated representative of all incidents including property damage, injuries, illnesses, near-miss events, environmental incidents, and regulatory agency inspections, and submission of written preliminary incident reports within 24 hours of serious incidents. The contractor must develop and maintain all federally, state, and locally required regulatory programs and ensure that safe work practices are properly planned and implemented across all work sites, including OCTA bus yards and rail facilities.
The specifications place primary responsibility on the contractor for identifying, preventing, and correcting all health, safety, and environmental hazards associated with its work, employees, and subcontractors. The contractor must provide all necessary tools, equipment, and safety protective devices required for compliant work performance and must instruct all employees and subcontractors in hazard identification and mitigation relevant to their assigned duties. Non-compliance with these HSE requirements, repeated unsafe acts or conditions, or serious violations may result in corrective action up to and including suspension of work, removal from site, or contract termination at OCTA's sole discretion. Additionally, the contractor must comply with all federal, state, county, municipal, and local environmental laws and regulations governing noise, air quality, water quality, storm water, hazardous materials, contaminated soils, and cultural or archaeological resources, as well as all applicable regulations related to hazardous and non-hazardous waste handling, recycling, and disposal.
