Services, Performance Questionnaire
The Department of Justice (DoJ), Federal Bureau of Investigation (FBI), Facilities Acquisition Unit-Huntsville (FAU-H) is in the process of selecting contractor(s) for award of an Aircraft Maintenance and Upgrades Service contract(s). The Contractor shall provide all supervision, labor, materials, parts, spare parts, incidentals, and other services necessary to provide Federal Aviation Regulation certified scheduled and unscheduled maintenance to maintain the Cessna Citation Encore and/or the Gulfstream aircraft.
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort.
We depend on information received from agencies such as yours, which have had first-hand experience with an offeror, for the evaluation of the offeror's performance on those contracts.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and e-mailed directly to me at JJTUCKER@fbi.gov and Corey Linnen, Contract Specialist, Contractor via email at clinnen@fbi.gov.
Please call Corey at (256) 213-4207 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.
Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.

Instructions for completing the Performance Questionnaire: Provide this questionnaire to the government agency or commercial organization (i.e. customer with knowledge of the Offerors past/present performance for aircraft maintenance and upgrades services.
The Department of Justice (DoJ), Federal Bureau of Investigation (FBI), Federal Acquisition Unit- Huntsville (FAU-H) is soliciting offers for Aircraft Maintenance and Upgrades Services contract(s) for the FBI’s Critical Incident Response Group (CIRG) Division located in the National Capitol Region
(NCR).
Please complete this questionnaire for past/present performance for aircraft maintenance and upgrades services for offeror who in the recent past or is currently providing you aircraft maintenance and upgrades services under a formalized contract. Handwritten responses are sufficient. If you need more space than that provided, please attach additional pages. Responses will be treated as source selection sensitive information. The offeror who provided the FBI your name as a reference was informed, via a solicitation provision, that by listing you as a reference and requesting your submission of the questionnaire, they are authorizing you to release information to the FBI relative to their past performance, whether it is positive or negative information.
The POC shall submit the completed questionnaire directly to the government contracting officer and contract specialist identified in the cover letter. The Contracting Officer will not accept completed questionnaires sent from the Offeror.

CODE PERFORMANCE LEVEL
E EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
VG VERY GOOD- The contractor’s performance meets contractual requirements and exceeds some (requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
M MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
U UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner.
