About this file

This document is a Reference Check Form (Attachment D) for RFP #S-63500-00013382 issued by DAS Procurement Services for Klamath and Malheur Watershed Helicopter Services. The form is designed for proposers to submit reference information from three professional references, including at least two non-ODFW (Oregon Department of Fish and Wildlife) references that can verify the proposer's capabilities. The references will be contacted and asked to rate the proposer on four key areas: overall quality of services, delivery of service, safety, and responsiveness to customer service issues, using a 0-5 scale where 0 represents "Not satisfied" and 5 represents "Extremely satisfied".

The reference check process involves the agency making three attempts to contact each provided reference, with proposers receiving zero points for any reference that cannot be reached. Each reference can contribute up to 5 points, with a potential grand total of 15 points possible across the three references. The form requires proposers to provide detailed contact information for each reference, including the reference entity, contact name, telephone number, and email address. This reference checking mechanism appears to be part of the evaluation process for selecting a helicopter services provider for watershed work in the Klamath and Malheur regions of Oregon.
