AMENDMENT 000002:

Purpose of this amendment is to answer the following question.

1. Question: Does your Professional Staff know if in fact this Physicist inspection is a ACR/Joint Commission requirement?

From Attachment: Amend-1 Attachment E - Scope of Work Temporary Mobile CT Trailer, Paragraph v.

Answer: For the CT mobile trailer, if they had it done prior, then all I need is the physicist report. In other words, they do not need to get a physicist to certify the unit for the placement at PIMC. As long as they did it and it's within the allowed timeframe per ACR/Joint Commission. Under these two accreditations they have to re-certify every year or two years. I refer them to the ACR/Joint Commision time frames.

Note: The New CT unit they are proposing and installing at PIMC, yes they need a medical physicist to certify the unit.

2. Question: Would it be possible to have the Contact information (Tel-Email) of the Physicist which PIMC currently Utilizes?

Answer:
