About this file

This is a Request for Proposal (RFP) issued by the Tempe Union High School District, located in the Greater Phoenix area of Arizona, for voluntary employee dental and vision insurance coverage. The District serves approximately 11,000 students across six comprehensive high schools and encompasses 162 square miles, including the City of Tempe, the Town of Guadalupe, the Gila River Indian Community, the Ahwatukee Foothills areas of Phoenix, and parts of Chandler. The District seeks proposals for three plan types: a voluntary pre-paid Dental Health Maintenance Organization (DHMO), a voluntary Dental Preferred Provider Organization (DPPO), and a voluntary vision plan to be offered from July 1, 2027 through June 30, 2032. The proposal deadline is 2:00 PM on Wednesday, November 4, 2026, with submissions required through the District's Procurement Portal. The RFP requires vendors to match current plan designs as closely as possible or provide detailed deviations if their designs preclude exact matching. Vendors must provide account management, customer service with toll-free access available Monday through Friday from 8:00 AM to 6:00 PM Arizona time, support for four days of annual open enrollment meetings in Tempe, bilingual communication materials, claims administration, provider network maintenance, eligibility and enrollment administration, and secured internet portal access for enrollment, eligibility, and claims data reporting.

Current coverage includes a Cigna DHMO and Delta Dental DPPO, with rates for the DHMO ranging from $9.45 for employee-only to $24.41 for employee plus family coverage as of July 1, 2026, and DPPO rates ranging from $37.21 to $97.31 for the same coverage tiers. Vision coverage through Davis Vision currently costs $4.78 for employee-only to $13.96 for employee plus family. Employees pay 100 percent of the premium cost for all coverage tiers. Eligible employees are active hourly workers regularly scheduled for a minimum of 30 hours per week, with benefits effective the first of the month following 30 days in a benefit-eligible position. Eligible dependents include lawful spouses and children up to age 26. The District requires all proposals be submitted net of commissions and notes that Segal, the consulting firm managing the procurement, is compensated through a consulting fee rather than commissions. Bidders requiring access to confidential historical claims data, enrollment information, and utilization reports for the past 36 months must execute a current Confidentiality Agreement or bid-related NDA with Segal before materials are released via secure email.
