About this file
This is a Disadvantaged Business Enterprise (DBE) and Non-DBE Subcontractor Utilization Form for RFP SUN260384, a consulting services solicitation issued by the City of Ocala for SunTran (Florida's public transit agency). The form requires prime contractors to disclose their anticipated use of certified DBEs as subcontractors and submit detailed information regarding all participating subcontractors, including their DBE certification status, scope of work, and contract amounts. Bidders must select one of three options: confirming that no subcontractors will be used, committing to a minimum 2.55% DBE utilization goal, or documenting good faith efforts if unable to meet the DBE threshold. The utilization form must be completed and included with the bid proposal package, and upon contract award, the form becomes an integrated component of the executed contract.
The form requires bidders to provide a comprehensive breakdown of all subcontractor work by DBE and non-DBE status, with corresponding dollar amounts totaling the bid proposal amount. All DBEs must be certified through the Unified Certification Program to qualify for DBE counting purposes. The City of Ocala's DBE Program defines positive good faith efforts, and failure to provide accurate information or exercise such efforts may result in the bidder being deemed non-responsive. The City Contracting Officer reserves the right to conduct an audit to verify submitted information. An authorized representative must certify the accuracy and completeness of the utilization form under penalty of misrepresentation.
