About this file
This is an RFP Submission Checklist for Professional Services opportunities at or above the formal RFP threshold for the City of Philadelphia Department of Public Health. The checklist serves as a compliance tool to ensure applicants submit complete, accurate, and timely proposals for the Department's Injury Prevention Program, which seeks initiatives supporting strategic priority areas to enhance the city's violence intervention capabilities. The document outlines submission requirements including deadline adherence, demonstration of how the applicant meets Scope of Work requirements detailed in Section II of the RFP, inclusion of a Cost Proposal, compliance with format and content standards specified in Section III, and submission through the electronic eContractPhilly platform under the correct opportunity number.
Applicants must submit financial documentation demonstrating capacity to perform the work, including audited or unaudited financial statements, bankruptcy disclosures covering the past five years, and IRS Form 990 for non-profit organizations. All proposals require submission of Appendix C: City of Philadelphia Tax and Regulatory Status and Clearance Statement, and applicants must disclose all information mandated under Chapter 17-1400 of the Philadelphia Code regarding local political contributions. Optional certifications include Appendix D for Local Business Entity or Local Impact Certification and Appendix E for LGBTQ Applicant Opportunity Data. The applicant must sign and submit the proposal electronically through eContractPhilly using an authorized signatory, ensuring that the registered entity name and Tax Identification Number match across the applicant profile, Appendix C, and W-9 form. The checklist was revised in July 2025.
