About this file
This is an RFP Submission Checklist document from the Commonwealth of Pennsylvania related to a Request for Proposal for forensic behavioral health evaluations for child welfare involved individuals and families. The checklist serves as a guide for qualified applicants preparing submissions in response to the RFP and outlines the required components and documentation that must be included in a complete proposal package. The document specifies mandatory submission requirements such as organizational documentation, qualifications, service delivery plans, and compliance certifications that applicants must compile and submit according to the RFP timeline and specifications.
The submission checklist establishes standardized requirements to ensure all proposals contain necessary information for consistent evaluation and comparison among applicants. The checklist typically includes verification of organizational eligibility, proof of insurance and licensure, detailed service plans addressing the forensic evaluation requirements, pricing schedules, references, and certifications regarding compliance with state regulations and procurement requirements. By providing this structured checklist, the Commonwealth ensures that all submitted proposals meet baseline completeness standards and contain the information necessary for the evaluation committee to assess applicant qualifications and capacity to deliver forensic behavioral health services to the child welfare system.
