About this file

This document is a Responsibility Inquiry form (Attachment G) for the Oregon Department of Administrative Services (DAS) Procurement Services, specifically for an OregonBuys Request for Proposal (RFP S-63500-00014394) related to Flow Simulation Software. The form is designed to assess the financial, operational, and legal responsibility of potential vendors bidding on the contract, with the agency reserving the right to request additional documentation such as financial statements, performance records, and other relevant information to evaluate the offeror's capability to fulfill contract requirements.

The inquiry covers multiple aspects of vendor responsibility, including past contract performance, legal history, financial stability, licensing, and compliance. Key areas of assessment include the vendor's ability to meet contractual obligations, history of contract completion, any criminal or civil charges, contract terminations, outstanding judgments, bankruptcy actions, required licenses and registrations, and pay equity certification. The form requires detailed responses to eight specific questions that comprehensively evaluate the vendor's reliability, financial health, and legal standing, with the understanding that failure to provide complete and accurate information may result in rejection of the proposal.
