About this file
The document is a Reference Check Form (Attachment D) for Oregon's Department of Administrative Services (DAS) Procurement Services, associated with RFP S-63500-00013989 for Bobcat GPS Collars. The form is designed for potential vendors to provide two professional references who will be evaluated on a 0-10 scale across multiple performance criteria, including service quality, timeline adherence, customer responsiveness, and technical performance of GPS collars.
The reference evaluation specifically focuses on technical performance metrics for GPS collars, including percentages of collars that stopped transmitting due to battery failure, firmware/software issues, and incorrect mortality identification. The Agency will make three attempts to contact each reference, and if unsuccessful, the vendor will receive a zero score for that reference. The form requires detailed contact information for each reference and is part of the overall vendor qualification process for the GPS collar procurement.
