About this file

This is a Proposer Information and Certification Sheet for the State of Oregon's RFP Oregon's Creative Economy Report (S-12300-00017003). The document serves as the certification and authorization form that prospective vendors must complete and submit as part of their proposal response. Proposers are required to provide their legal business name, address, state of incorporation, entity type, contact information, and Oregon Business Registry Number if applicable. By submitting this certification, proposers confirm their understanding and acceptance of all RFP requirements, agreement to be bound by the contract terms and conditions in Attachment F and any addenda, and that their proposal constitutes a firm offer valid for 30 days following the closing date. If awarded the contract, proposers commit to performing the scope of work and meeting the performance standards as defined in the final negotiated contract.

The certification sheet requires proposers to attest to multiple compliance and eligibility requirements, including current status with Oregon state and local tax obligations, non-discriminatory employment and subcontracting practices, prevention of sexual harassment and discrimination (required for contracts anticipated at $150,000 or more), wage and compensation equity compliance under ORS 652.220, broadband service provision compliance if applicable, and verification that neither the proposer nor its employees, agents, or subcontractors appear on federal exclusion lists maintained by OFAC or SAM. Proposers must also certify the absence of conflicts of interest with the State, confirm that all proposal contents are truthful and accurate, and acknowledge that false or fraudulent statements may result in liability under the Oregon False Claims Act. An authorized representative's signature and date are required on the certification sheet, with the understanding that these certifications supplement any additional certifications required at contract execution.
