About this file
This document is an Attachment E to RFP 6314 issued by the Oregon Health Authority, Public Health Division, for panel survey services supporting the Oregon Behavioral Risk Factor Surveillance System State Supplemental Panel Survey. The attachment functions as a Certified Disadvantaged Business Outreach Plan form that proposers must complete and submit with their proposals. The form requires proposers to disclose their own certification status under Oregon's certified firm program (which includes minority-owned businesses, woman-owned businesses, service-disabled veteran-owned businesses, and emerging small businesses), list past contractual relationships with certified and non-certified firms, and identify any anticipated subcontracting opportunities. Proposers who anticipate subcontracting must describe the steps they will take to solicit certified firms, detail the specific subcontracting opportunities and their approximate dollar values, and confirm their willingness to report subcontractor identities and values to the Oregon Certification Office for Business Inclusion and Diversity (COBID).
The outreach plan submission is mandatory for all proposers but is explicitly stated as non-scored and non-evaluated in the procurement process. Oregon law requires that certified firms be provided equal opportunity to compete for and participate in subcontracts funded with state money. The form serves as a mechanism to ensure compliance with Oregon Revised Statutes 200.055 and to promote diversity in state contracting by documenting proposers' good faith efforts to engage disadvantaged business enterprises in the supply chain.
