About this file

This document is a Tobacco, Alcohol and Cannabis Certification Sheet (Attachment H) issued by the Oregon Health Authority for RFP 6300: Commercial Tobacco Prevention and Cessation Communications Strategy and Services. The certification sheet establishes mandatory compliance requirements for proposers responding to this request for strategic communications services supporting Oregon's tobacco prevention and cessation programs, including the Smokefree Oregon social-marketing brand and culturally specific campaigns managed by the Public Health Division's Tobacco Prevention and Education Program.

Proposers must certify compliance with six key requirements: (1) acknowledgment that false or fraudulent statements constitute claims under the Oregon False Claims Act with associated penalties; (2) implementation of a 100% Tobacco-free Workplace Policy or a plan to develop one within six weeks of contract execution; (3) good faith efforts to offer comprehensive tobacco cessation benefits to employees; (4) confirmation of no direct or indirect organizational or business relationships with entities involved in tobacco, alcohol, or cannabis production, distribution, promotion, or sale, and certification of no revenues from such sources within the preceding two years; (5) commitment to refrain from accepting revenues from tobacco, alcohol, or cannabis manufacturers or promoters during the agreement period; and (6) acknowledgment that these certifications supplement additional certifications required at contract execution. All certifications require authorized signature and dating by the proposer.
