About this file
This file is a premium rate schedule appendix for RFP 31786-00192 issued by the State of Tennessee for a voluntary, fully-insured vision insurance plan serving state employees, local education and local government employees, retirees, and dependents across approximately 175,000 members. The RFP solicits proposals for two distinct benefit options—Basic Vision Insurance Program and Expanded Vision Insurance Program—with statewide and national network coverage spanning Tennessee's 83 counties plus all 50 states and the District of Columbia. Responses were due November 6, 2026, with contract performance spanning January 1, 2028, through December 31, 2032. A pre-response conference was scheduled for August 21, 2026, with written questions due September 4, 2026, and a second round of questions due October 2, 2026. Technical evaluation completion was targeted for December 17, 2026, with cost proposal opening on December 18, 2026, negotiations scheduled for January 6-8, 2027, and Notice of Intent to Award on January 28, 2027. All proposals were required to remain valid for at least 120 days following cost proposal opening.
The premium rate schedule establishes guaranteed monthly rates per subscriber for five coverage categories across both benefit plans for the five-year contract period from January 1, 2023, through December 31, 2027. Basic Vision Insurance rates range from $3.18 per month for Employee/Retiree coverage to $9.33 for Employee/Retiree & Spouse & Children coverage, with rates held constant across all five contract years. Expanded Vision Insurance rates are approximately double, ranging from $6.30 to $18.54 per month across the same five coverage categories and contract years. The current incumbent vendor is EyeMed Vision. Evaluation criteria weight general qualifications and experience at 10 points maximum, technical qualifications and experience at 20 points maximum, network analysis at 30 points maximum, and cost proposals at 40 points maximum, with award made to the responsive and responsible respondent with the best overall combination of attributes rather than necessarily the lowest cost.
