About this file
This is a Schedule B Pricing document for Request for Proposal No. 260000000604 issued by the State of Michigan for Temporary Medical Staffing Services. The RFP seeks pricing submissions for a comprehensive range of medical and healthcare positions, including physicians, nurses (registered, licensed practical, and nurse managers), allied health professionals (such as physical therapists, occupational therapists, and respiratory therapists), mental health professionals (psychiatrists, psychologists, and licensed clinical social workers), dental professionals, and various support staff positions including medical assistants, phlebotomists, and pharmacy assistants. Contractors must submit pricing schedules in modifiable formats such as Microsoft Word or Excel, with the option to submit additional pricing in non-modifiable formats such as PDF, and failure to complete the pricing schedule as requested may result in proposal disqualification.
Pricing proposals must include all costs associated with staffing placement, including one-time charges, set-up fees, shipping, and any other potential expenses the contractor may charge the State. Contractors are required to provide three pricing components for each position: the Hourly Rate (the amount employees will receive in their paychecks), the Billable Rate (the amount charged to the State for placing an employee), and the Mark-up percentage (the percentage added to the hourly rate to cover all placement-related expenses). Additionally, contractors are encouraged to offer quick payment discounts, specifying the percentage discount if invoices are paid within a specified number of days of receipt. The pricing document also includes a separate line item for Michigan Department of Corrections Computer-Based Training at a fixed rate of $10.00 per person per module with no contractor markup allowed.
