About this file
This is Amendment 2 to a pricing schedule (Schedule B) for RFP# 260000000604 issued by the State of Michigan for Temporary Medical Staffing Services. The request solicits pricing proposals from contractors to provide temporary medical staffing across a comprehensive range of healthcare positions, including physicians, nurses, therapists, technicians, and support staff such as dentists, psychiatrists, pharmacists, and various specialists. Contractors must submit detailed pricing schedules in modifiable format (Microsoft Word or Excel) with the option to provide a non-modifiable PDF copy. The pricing schedule identifies over 45 distinct healthcare positions requiring hourly rate, billable rate, and markup percentage submissions.
Contractors must provide all-inclusive pricing that covers one-time charges, setup fees, shipping, handling, and all expenses associated with temporary employee placement. The document encourages quick payment discounts and requires contractors to certify pricing was developed independently without consultation with competitors. The State reserves the right to standardize hourly rates across awarded contracts for specific positions (for example, establishing uniform RN rates across all contracts) with initial determinations made during evaluation and award and subsequent updates issued via change notice. Computer-based MDOC training is priced at $10.00 per person per module with no contractor markup allowed. Failure to complete the pricing schedule as requested may result in proposal disqualification.
