About this file

This document is a Schedule of Benefits attachment for the DHMO S200B Access Enhanced plan option under Palm Beach County's Multiple Option Dental Program RFP 26-041. The program covers county employees, Palm Tran employees, and Supervisor of Elections staff, with proposals due July 6, 2026. The contract period runs from January 1, 2027 through December 31, 2029. The DHMO S200B Access Enhanced plan (Option II) provides comprehensive dental coverage including diagnostic, preventive, basic restorative, oral surgery, and limited prosthodontic services through participating in-network providers. Members enjoy no waiting periods, no deductibles, and immediate eligibility upon the effective date of coverage. Most diagnostic and preventive services are provided at no charge to members, including periodic oral evaluations, radiography, prophylaxis, fluoride treatments, and sealants. The plan covers basic restorative services such as amalgam and composite fillings, simple extractions, and basic periodontal services at specified copayments ranging from $0 to $250 for in-network providers.

The S200B Access Enhanced plan establishes distinct copayment structures for in-network and out-of-network reimbursements across all service categories. Member copayments for in-network services vary by procedure code, with preventive care at $0 copayment, diagnostic services ranging from $0 to $180, restorative procedures from $20 to $350, endodontic services from $10 to $535, periodontal services from $40 to $502, and oral surgery from $10 to $800. Out-of-network services are reimbursed at a schedule rate to the member. The plan includes specific limitations and additional fees: bitewings are limited to one set per twelve consecutive months; prophylaxis and periodontal maintenance limited to one per six-month period; fluoride treatment limited to one per twelve months; sealants limited to one per tooth per three years for children under sixteen; and copayments marked with asterisks exclude material and laboratory fees with additional patient costs up to $225 for denture laboratory work and $155 for crown laboratory fees. Members must verify all fees with participating providers or the Member Services Department at 1.877.760.2247 prior to treatment.
