This is a Bidder Requirements and Qualifications Form (Attachment C) for RFP 25 8428 issued by the Orlando Utilities Commission (OUC) in Florida for Occupational Medical Services. The solicitation seeks one or more qualified suppliers to provide comprehensive occupational medical services including pre-employment post-offer examinations, general medical examinations, fitness-for-duty evaluations, exposure response, random drug and alcohol screenings, and Medical Director consultation. Proposals were due July 17, 2025, at 2:30 PM EST. The contract term is anticipated to be five years with one potential one-year renewal option. The form establishes mandatory minimum qualifications that proposers must meet to avoid disqualification, including five years of occupational medicine experience, minimum two locations in each of six specified Florida counties (Brevard, Orange, Osceola, Seminole, Volusia, and Lake), proper licensure documentation for all assigned physicians and health professionals, at least one Board Certified Occupational Medicine physician on staff, and at least one out-of-state location for pre-employment physicals and drug screens.

The qualification form requires extensive documentation and responses across multiple categories including bidder information, company registration status, organizational structure, facility details, hours of operation, employee counts, service agreements history, and three pre-verified client references from the past five years demonstrating similar scope and volume. Bidders must provide detailed information on subcontractors, physician assignments with certifications and tenure, billing statements, service level agreements, drug testing capabilities (DOT and non-DOT), electronic scheduling and reporting systems, chain of custody procedures, and compliance with federal and state testing laws. The form requires confirmation of specific service capabilities such as customized monthly reporting, 24/7/365 web portal access, real-time status updates, nationwide coverage for drug screenings with details on out-of-state facilities by state, and acceptance of major credit cards. No specific set-aside for disadvantaged enterprises is mentioned, with the M/WBE goal listed at 0%. The RFP does not require bid bonds or performance bonds, and payment terms include options for 1% discount if paid within 20 days or 3% discount if paid within 10 days, with standard net 30-day payment available.
