About this file
This is an Exhibit H compliance document for the Washington State Health Care Authority's RFP 2025HCA5 for Fully-Insured Medical Plans. The document serves as a response template for Question 6.19 within the Written Proposal Phase 1, requiring bidders to demonstrate compliance with Bree Collaborative guidelines on Complex Discharge and Social Determinants of Health (SDOH) and Health Equity. Bidders must complete two assessment tabs: the first addresses Complex Discharge implementation, requiring bidders to indicate full compliance, partial compliance, or plans for compliance across eight key recommendations including adoption of common discharge definitions, development of documented discharge barrier processes, member identification and tracking systems, and maintenance of adequate post-acute provider networks. The second tab addresses SDOH and Health Equity implementation, requiring similar compliance indicators across ten recommendations including embedding equity principles into organizational mission and values, collecting Race, Ethnicity, and Language (REaL) data, developing processes for sexual orientation and gender identity (SOGI) and disability data collection, stratifying HEDIS measures to identify disparities, and coordinating with community-based organizations and state agencies on social determinant goals.
The document provides specific guidance that bidders must mark compliance levels with an "X" in corresponding cells and include written descriptions of plans for achieving compliance if not already fully or partially implemented. Bidders are directed to reference the Bree Collaborative's published recommendations on Complex Discharge and Social Needs Screening, as well as Foundation for Health Care Quality standards. The compliance assessment is mandatory for participation in this RFP and reflects the HCA's emphasis on health equity, care coordination for complex patients, and social determinants of health integration within the proposed fully-insured health plans.

| Adopt the common definiton for complex patent discharge: Patents who are medically ready to be transferred outside of an acute care settng but are unable to do so due to discharge barriers. | ||||
| Develop process of receiving standard, documented discharge barrier informaton from the acute care settng as outlined in Appendix B: Standard Discharge Barrier List and communicate to the dedicated complex discharge team or lead. | ||||
| Have a system to identify and track members that qualify as a complex patient discharge. |

Screen for and track rates of Social Determinants of Health (SDOH) screening and referral for members, and stratify by Race, Ethnicity and Language (REaL) data to identify disparities.
Develop pathways to address identified disparities. Follow Foundation for Health Care Quality Recommendations on Social Needs Screening and Social Needs Intervention to prioritize screening for social needs.
Maintain adequate network of post-acute providers for every hospital referral region based on historic need for post-acute bed placement.
o Regularly verify network is adequate by analyzing utilization numbers.
o Identify post-acute providers that can accept acute care discharges on weekends and holidays and communicate this information to discharge planning teams.
