About this file

This is an Exhibit G Plan Design Outline for a Request for Proposals (RFP 2025HCA5) issued by the Washington State Health Care Authority (HCA) to procure fully-insured health plans for the Public Employees Benefits Board (PEBB) and School Employees Benefits Board (SEBB) programs. Bidders must propose up to three health plan designs (Plans A, B, and C) that will be offered to non-Medicare PEBB and SEBB subscribers and their dependents. All proposed health plans must maintain a federal Actuarial Value (AV) between 80% and 92%. Plan A must match the Uniform Medical Plan (UMP) Classic design in all aspects including deductibles, maximum out-of-pocket limits, copays, coinsurances, visit limitations, and other accumulators, though bidders may use their own drug formularies for pharmacy tiers. Plan B requires bidders to match specified deductible, maximum out-of-pocket, and pharmacy structure elements while retaining discretion over coinsurance and copay amounts. Plan C provides the most flexibility, requiring only adherence to medical and pharmacy deductible and maximum out-of-pocket guidelines, with bidders having discretion over most other plan design elements. The identical plan designs must be offered to both PEBB and SEBB, though each program will be separately rated due to distinct risk pools.

Bidders must complete detailed spreadsheet tabs documenting specific benefit designs, cost-sharing structures, visit limitations, and exclusions for each proposed plan, along with pharmacy tier information and preferred drug list details for a comprehensive list of specialty drugs and biologics. Final plan design and the number of plans accepted from each bidder will be determined during contract negotiations, with HCA retaining sole discretion to determine which health plans are presented for final approval by both the PEBB Board and SEBB Board. The document specifies that covered services must include mandatory offerings such as contraceptives, diabetes control and prevention programs, preventive care, hearing aids, doula services, and gender-affirming care at specified cost-sharing levels. Mental health parity compliance is required, particularly for neurodevelopmental therapy services. Bidders must attest to matching UMP Classic exclusions as outlined in the UMP Certificate of Coverage and provide complete formulary information, including tier assignments, cost-sharing amounts, and prior authorization requirements for all covered drugs.

Instructions

Plan Design Bidding General Instructions:
1.Bidders must thoroughly review the Draft Contract Attachment 3: Statement of Work, 1. PLAN DESIGN FOR FULLY INSURED HEALTH PLANS before completing Exhibit G_Plan Design Outline_RFP.

NOTE: Fully-insured Health Plan(s) will be offered to non-Medicare Public Employees Benefits Board (PEBB) and School Employees Benefits Board (SEBB) Program Subscribers and their Dependents. The Health Plans offered to PEBB must be identical in plan design to the Health Plans offered to SEBB but, because each program has a separate risk pool, the plans will each be separately rated for the PEBB program and the SEBB program.

2.All Health Plans offered to HCA must maintain a federal Actuarial Value (AV) of 80% to 92%.
3.The plans proposed in this Exhibit are all the benefit designs we will evaluate for the RFP. Plan A will be evaluated primarily for rates purposes. Other plans will be evaluated on cost, design, and how they fit with our larger portfolio.
4.Final plan design and number of plans accepted from Bidder will be determined during contract negotiations.

NOTE: HCA, at its sole discretion, will determine the Health Plans to be presented for final approval of the Boards. All Health Plans must be approved by both the PEB Board and the SEB Board and must be open only to PEBB and SEBB Members.

Tab Titled “Plan A” For the purpose of HCA evaluation, Bidder’s Health Plans designated as Plan A must match Uniform Medical Plan (UMP) Classic in all aspects of deductible, maximum out of pocket, copays and coinsurances, visit limitations, and other accumulators. Pharmacy tiers are also defined, but Bidders are not required to match the drugs assigned to each tier in the UMP Formulary. Reviewing the UMP Certificate of Coverage (CoC) may be helpful in completing this tab. It is available at UMP Classic CoC.

Please use the Tab A in the Exhibit G_ Plan Design Outline_RFP to add the details of Bidder’s Plan A for PEBB and SEBB. Gray boxes in the spreadsheet indicate where Bidder is to fill in values.

Tab Titled “Plan B” Bidder’s Health Plan designated as “Plan B” will be required to match certain elements of deductible, maximum out of pocket, and pharmacy structure that HCA has laid out in Exhibit G_ Plan Design Outline_RFP. However, other portions of plan design like amounts of coinsurance and copays are left open to Bidder’s offering. Gray boxes in the spreadsheet indicate where Bidder is to fill in values.

Please use the Tab B in Exhibit G_ Plan Design Outline_RFP to add the details of Bidder’s Plan B for PEBB and SEBB.

Tab Titled “Plan C” Bidder’s Health Plan designated as “Plan C” will be required to use the medical and pharmacy deductible and maximum out of pocket guidelines in Exhibit G_ Plan Design Outline_RFP, but Bidder may decide most other elements of plan design at Bidder’s discretion. Gray boxes in the spreadsheet indicate where Bidder is to fill in values.

Please use the Tab C in Exhibit G_ Plan Design Outline_RFP to add the details of Bidder’s Plan C for PEBB and SEBB.

Tab Titled “Rx Class & Tiers” This tab is intended to provide information to HCA about the preferred drug list of each Bidder. Please complete the required fields with the requested details about drug product name, proposed tier, cost share, and whether prior authorization is required for the drug. Gray boxes in the spreadsheet indicate where Bidder is to fill in values.
