About this file

This document is Addendum #2 to Request for Grant Application (RFGA) 6030 for Public Health Equity Grants, issued by the Oregon Health Authority (OHA) on August 15, 2025. The addendum provides clarifications, amendments, and answers to questions about a grant opportunity for community-based organizations (CBOs). The funding period will run from the award date to June 30, 2027, with payments anticipated to start in Spring. Applicants can apply to multiple funding categories, including Commercial Tobacco, Communicable Disease, Community Connection, and Community Resilience, with each category having specific priority health outcomes and eligible activities.

The grant has several key financial and eligibility parameters: there is a $750,000 maximum award per organization, applicants must be 501(c)(3) organizations, and funding is a mix of state and federal sources. Allowable expenses include professional development, staff training, community education, participant incentives, and specific program-related costs. Applicants must submit two letters of support, provide detailed project plans explaining their intended outreach population, and demonstrate alignment with the program's priority health outcomes. The application process involves registering through Webgrants, with organizations able to apply to multiple categories by completing separate project sections for each.
