About this file
This is an insurance requirements document for the County of Volusia and SMA Healthcare, Inc. related to construction services for flood mitigation improvements at Ridgewood in Volusia County, Florida. The project is funded through a U.S. Department of Housing and Urban Development Community Development Block Grant for Disaster Recovery (CDBG-DR) to support recovery efforts from Hurricane Ian damage sustained in September 2022. The document establishes comprehensive insurance coverage requirements that contractors must maintain throughout the project duration, which extends until equipment has successfully completed testing, commissioning, and acceptance by SMA Healthcare.
The insurance requirements include workers compensation with Florida statutory coverage and waiver of subrogation; commercial general liability with limits of $1,000,000 per occurrence and $2,000,000 general aggregate; auto liability with combined single limit of $1,000,000; excess liability of $1,000,000 per occurrence/aggregate; contractors pollution liability of $1,000,000 per incident/aggregate; installation floaters covering all-risk wind, hail, and named storms with flood coverage if applicable in FEMA-designated special flood hazard areas; and professional liability of $1,000,000 per incident with $1,000,000 aggregate. All policies must name County of Volusia and SMA Healthcare as additional insureds using specified ISO forms, maintain primary and non-contributory status, include waivers of subrogation in favor of both entities, and provide thirty days written notice of cancellation to the County of Volusia Purchasing & Contracts Division in DeLand, Florida.
