About this file

This document is a Required Certifications form for the Department of Children and Families (DCF) Invitation to Bid (ITB) 2526 010 for Milk for State Mental Health Treatment Facilities, effective June 14, 2024. The form is a comprehensive vendor certification document requiring potential respondents to complete detailed organizational information and provide certifications across multiple compliance areas. Vendors must provide their official business details, primary and alternate contact information, and affirm compliance with various state and federal statutory requirements.

The certification document requires vendors to attest to several key compliance areas, including no conflicts of interest, no collusion in bidding, prohibition of gratuities, and confirmation of not being listed on various vendor exclusion lists. Vendors must certify they are not engaged in boycotts, are not on convicted or discriminatory vendor lists, and comply with scrutiny requirements related to business operations in specific countries. The document also offers optional tiebreaking certifications for minority businesses, veteran-owned businesses, Florida businesses, and drug-free workplaces, which could provide competitive advantages in bid evaluations.
