About this file

This document is a Required Certifications form for Invitation to Bid (ITB) 2526 010 by the Florida Department of Children and Families (DCF) for milk procurement for state mental health treatment facilities, effective June 14, 2024. The form is a comprehensive vendor certification document requiring potential respondents to complete detailed organizational information and attest to various legal and ethical compliance standards.

The certification form requires vendors to verify their status across multiple vendor lists, confirm they are not on restricted lists such as the Convicted Vendors List or Discriminatory Vendors List, and certify compliance with Florida Statutes regarding conflicts of interest, collusion, gratuities, and business operations. Vendors must also disclose whether they qualify for tiebreaker provisions, such as being a Certified Minority Business Enterprise, Certified Veteran Business Enterprise, or having a Drug-Free Workplace. The document provides optional checkboxes for vendors to claim potential preferential status in bid evaluation scenarios, with the understanding that failing to submit certifications by the response due date waives consideration of these tiebreaker advantages.
