About this file
This document is a Request for Proposal (RFP) issued by the Davis School District Purchasing Department located in Clearfield, Utah. The RFP is a standard solicitation form designed for vendors to complete when submitting a proposal for goods or services, with fields for company details, business type, contact information, and certification of compliance with Utah state regulations.
The form includes critical state-mandated certifications, specifically requiring vendors to confirm they are not engaged in economic boycotts and are complying with Utah Code 63G-27-102. Vendors must certify they are not boycotting the State of Israel or companies involved in various sectors such as traditional energy, mining, agriculture, firearms manufacturing, or those not meeting certain environmental or healthcare access standards. The document provides checkboxes for vendors to affirm these certifications and requires an authorized representative's signature, demonstrating the state's commitment to ethical procurement practices.
