About this file

This is an Acceptance of Offer Form issued by the City of Phoenix, a municipal corporation in Arizona, for use in formalizing contractor agreements. The form documents the City's acceptance of a contractor's offer and binds the contractor to provide the materials or services outlined in the associated contract and solicitation documents, including all terms, conditions, specifications, and amendments. The contract becomes effective upon the City Manager's signature and the City Clerk's attestation. Contractors are explicitly cautioned not to commence any billable work or provide materials or services until receiving official purchase order or contract documentation from the City. The form requires completion of contract number, acceptance director information, and signatures from appropriate City officials, with City Attorney approval required only if the form document is altered from its standard version.

This form serves as the formal acceptance mechanism for the City of Phoenix Finance Department's Title and Escrow Services solicitation, which was released on March 24, 2026, with sealed proposals due by April 22, 2026, at 2:00 p.m. The resulting five-year contract will commence on or about December 1, 2026, contingent upon City Council award and execution. The City seeks qualified contractors to provide comprehensive title and escrow services, including title reporting, escrow agency functions, and title insurance policy issuance. Offerors must demonstrate a minimum of five years of professional experience in Arizona title and escrow services and maintain appropriate licensure from the Arizona Department of Insurance and Financial Institutions. The evaluation criteria allocate 330 points for capacity, 250 points for method of approach, 220 points for qualifications and experience, and 200 points for pricing. All prices are firm and fixed for the initial two years, with annual increases permitted thereafter upon written submission with 60 days' notice. The City intends to award contracts to multiple firms and requires all offerors to maintain professional liability insurance with limits of $1,000,000 per claim and $1,000,000 aggregate, along with standard commercial general liability, workers' compensation, and automobile liability coverage.
