About this file
This document is a Recycled Material Content Questionnaire and Affidavit from Collin County, designed to track and verify the recycled material content of products being procured. The form requires suppliers to reference the latest Consolidated Recovered Materials Advisory Notice (RMAN) from the EPA's Comprehensive Procurement Guideline (CPG) and complete a detailed table for each product, indicating the section of RMAN that includes the product, the percentage of recovered and post-consumer material, and whether the product meets specific procurement recommendations.
The questionnaire obligates suppliers to swear that the information provided is accurate and that they will maintain compliance throughout the contract term. Suppliers must be prepared for potential audits by Collin County and agree to notify the county within 30 days of any changes in the recyclable content of their products. The form includes signature lines for an authorized representative, requiring their printed name, title, and the date of submission, thereby creating a legally binding document that supports the county's preference for environmentally responsible purchasing.
