About this file

This document is a Conflict of Interest Questionnaire (Form CIQ) from the Texas Ethics Commission, designed for vendors doing business with local governmental entities. The form is part of Chapter 176 of the Local Government Code and requires vendors to disclose any potential conflicts of interest when entering into business relationships with local government officers or their family members. The questionnaire must be filed within 7 business days of becoming aware of facts that necessitate the statement, such as beginning contract discussions, submitting a proposal, or recognizing a business or family relationship with a government officer.

The form specifically requires vendors to disclose any employment or business relationships that could result in taxable income exceeding $2,500 in a 12-month period, or gifts to a local government officer or their family members valued over $100 in the same period. Vendors must provide details about any such relationships, including whether the local government officer or their family member is receiving or likely to receive taxable income from the vendor, and vice versa. The document emphasizes that knowingly violating these disclosure requirements is considered a misdemeanor, underscoring the importance of transparency and ethical business practices in local government transactions.
