About this file
This document is a Past Performance Questionnaire template used for evaluating contractor performance on federal contracts, specifically for RFP 75F40126R00020 (OSIST OC-2026-132216).
The questionnaire is designed to be completed by government references who have direct knowledge of a contractor's prior performance. It captures contract details including contract number, total value, period of performance, and whether the contractor served as prime or subcontractor. The assessment comprises seven rated elements: (1) Cost/Price Control—evaluating cost management and adherence to agreed-to pricing; (2) Schedule Adherence—measuring compliance with delivery schedules and timeliness of deliverables; (3) Performance—rating compliance with contract requirements and statements of work; (4) Quality—assessing adherence to quality assurance plans and performance-based requirements; (5) Business Relations—evaluating cooperation, personnel stability, and responsiveness to change management; (6) Personnel Qualifications—rating the contractor's ability to staff and retain qualified personnel; and (7) Overall Performance Rating. Each element uses a five-point scale: Very Good (VG), Satisfactory (S), Marginal (M), Unacceptable (U), and Not Applicable (NA). Respondents must provide supporting narratives for each rating and answer supplemental questions regarding contractor strengths, weaknesses, and likelihood of future contract award. The questionnaire concludes with a certification statement affirming the accuracy of responses.
