About this file
This is a quote submission template provided by the government for solicitation W912LN-25-Q-A006, which seeks full food service support for the Vermont Regional Training Institute from January-June 2025. The template is organized into three volumes: Experience, Past Performance, and Price.
The template allows vendors to document up to five relevant contracts from the past five years for Experience, and up to three contracts from the past three years for Past Performance evaluation. Experience factors focus on food preparation/quality, menu standards, meal service, and food safety/sanitation. The Price Volume includes 25 Contract Line Item Numbers (CLINs) covering hot breakfast, bag lunch, IDT hot lunch, hot lunch, and hot dinner services across five performance periods: January (1,408 hot breakfasts/dinners, 128 bag lunches, 52 IDT hot lunches, 1,216 hot lunches), with similar quantities repeating in February, March, May and June 2025. While use of the template is not mandatory, it is designed to help vendors ensure complete submissions and avoid competitive disadvantages from missing information.

QUOTE SUBMISSION TEMPLATE
You are strongly encouraged to submit quotes using this template. It is not mandatory, but tracking everything the Government has asked for can be challenging, and an otherwise-attractive, competitively priced offer may be at a disadvantage if you fail to include all requested information. You may submit the form as a Word document or print it, fill it out by hand, and submit a PDF or photo. Most cell phones have a document scanning app included.

VOLUME 1: EXPERIENCE
Submission of Experience information is not mandatory. However, because Experience is one of the evaluation factors, failure to include Experience information may put your quote at a competitive disadvantage relative to another vendor that does include relevant experience. You would receive a neutral rating for Experience, while the other vendor would receive a positive rating for this factor.
Note: “Experience” is what work the offeror has done in the past. “Past Performance” is how well they did it. For Volume 1, just identify relevant contracts (contracts that are similar in scope, magnitude of effort, and complexity to the work in this solicitation). Performance evaluation is not part of this volume.
You may list up to five (5) contracts performed within the last five (5) years. If you list fewer than 5, you may delete, cross out, or discard tables you do not use. The question about Factors 1 through 4 refers to the work described in the referenced Performance Work Statement (PWS) paragraphs:
Factor 1: Food Preparation & Quality. PWS paragraph(s) 5.1 thru 5.1.2.
Factor 2: Menu Standards. PWS paragraph(s) 5.2 thru 5.2.3.
Factor 3: Meal Service. PWS paragraphs 5.3 thru 5.3.5.
Factor 4: Food Safety, Sanitation & Cleaning. PWS paragraphs 5.4 through 5.4.4, and 5.5.3.
Do not include descriptions of work not relevant to catering, food service or management of catering/food service contracts. If you include information regarding more than five (5) contracts, the Government will consider only the first five contracts listed.

VOLUME 2: PAST PERFORMANCE
Submission of Past Performance information is not mandatory. However, Past Performance is one of the evaluation factors. Again, if you do not submit Past Performance information your quote may be at a competitive disadvantage relative to another quote that contains favorable past performance information.
If the offeror wishes to have any of the contracts listed in Volume 2 considered for purposes of Past Performance evaluation (how well the work was performed, not just that it was undertaken), the offeror may submit the following additional information regarding up to three (3) of the contracts identified in Volume 2 that occurred within three (3) calendar years prior to this solicitation. Delete, cross out or discard any tables you do not use.
Offeror’s Past Performance Past Performance Reference 1:

A description of the scope, magnitude of effort, and complexities of effort called for on the contract, sufficient to support a conclusion that the offeror’s performance on that contract has relevance to the current effort.
Narrative self-assessment of the offeror’s performance with respect to Factors 1-4.
