About this file
This is a Question and Answer document for Illinois Department of Human Services' Invitation for Bid (IFB) for carpet removal and epoxy flooring installation at the Alton Forensic Center, 4500 College Ave., Alton, IL. The project encompasses approximately 16,230 square feet across patient and non-patient areas, with work organized in multiple phases: Units A and C will each require two phases, Unit B will be completed in one phase, and remaining areas will be divided into several phases. Work hours are Monday through Friday, 7:00 AM to 4:30 PM, with additional hours requiring facility approval. The scope includes comprehensive substrate preparation, crack filling for cracks 1/16 inch or larger, mechanical profiling, and installation of seamless epoxy flooring with a finished thickness of 50 MILS and integral cove base (3/4 inch) extended 4 inches up walls in all designated areas. The epoxy system consists of a primer, base coat, and matte-finish topcoat with a standard flake formula applied to all areas except patient rooms. A board sample will be required for approval. The contractor must attend the mandatory pre-bid conference; bids are due November 5, 2026, with questions due by 5:00 PM on October 23, 2026. The contract term extends from the last-dated signature through June 30, 2027.
The project is subject to Illinois prevailing wage requirements. Bidders must attend the pre-bid conference in person; sub-contractors cannot sign in on behalf of the bidding vendor, and this attendance requirement is a condition of bid responsiveness though not part of the formal contract scope. A Business Enterprise Program (BEP) utilization goal of 5% applies, requiring submission of a completed BEP Utilization Plan with documented BEP-certified subcontractor participation or good faith efforts as a condition of responsive bid status. Contractors must perform a minimum of 51 percent of total contract work with permanent, full-time W-2 employees employed for at least 30 consecutive days prior to bid submission; temporary labor, 1099 contractors, and staff augmentation do not qualify toward self-performance. General Commercial Liability insurance of $1,000,000 per occurrence and $2,000,000 annual aggregate, plus $1,000,000 Auto Liability per occurrence, are required, with the State named as additionally insured. A surety bond in an amount not less than the bid amount from a company rated at least A is mandatory. AMHC will be responsible for moving furniture as needed. Bidders should estimate all costs related to substrate preparation defects, moisture concrete readings outside system specification, and crack filling; the document directs contractors to bid the amount necessary for these contingencies rather than accepting predetermined limits.
