Project Status: Select whether your project/activity is an entirely new project or an extension of an existing project:

☒New CIO Project/Activity

☐ CIO Existing Project/Activity (Expansion/Change of Scope)

Project Description: Provide a description of the project including a justification for the project’s bona fide need in the space below:

APHL and CDC will work collaboratively, with OLDCC, the Guam Office of the Governor, and the Guam DPHSS, to provide technical assistance on the laboratory design and other administration efforts related to the commissioning of the new Guam Public Health Laboratory. CDC will provide technical assistance, support and administrative oversight through the cooperative agreement with the Association of Public Health Laboratories (APHL).

Activities: Describe activities to be carried out in this project as well as their associated deliverables:

1). APHL will collaborate with HDR, through a formal agreement, to produce a laboratory design for the Guam laboratory to include laboratory program requirements and overall schematic design 2). Working with HDR, provide assistance and review the Guam Office of the Governor’s Sub-Recipient Business Case Analysis (analysis to determine which Government of Guam organization should be the Sub-Recipient) and provide comments and recommendations to CDC and OLDCC 3). Along with HDR, establish a Guam Public Health Laboratory Working Group or Groups comprised of subject matter experts from DOD, HHS, and CDC ), US Department of Health and Human Services (HHS), Centers for Disease Control and Prevention (CDC), Guam Department of Public Health and Social Services (DPHSS), Guam Office of the Governor, and the Association of Public Health Laboratories (APHL), inclusive of a Design Agent of Record, with specialized design expertise in public health laboratories 4). Through a formal agreement with HDR, APHL and HDR shall manage the Project Budget Estimate, which captures all project costs, and provide updated Budget Reports throughout the entire project performance period (all Phases) 5). APHL and HDR, shall conduct a Design Update Meeting at the end of each Design Phase that provides an executive overview of the design evolution and/or refinement and current project budget to the Working Group(s) and to relevant Guam stakeholders.
