SECTION 2: PROJECT SUMMARY

2.1 – Project Description

Project Status: Select whether your project/activity is an entirely new project or an extension of an existing project:

☒New CIO Project/Activity

☐ CIO Existing Project/Activity (Expansion/Change of Scope)

Project Description: Provide a description of the project including a justification for the project’s bona fide need in the space below:

COVID-19 has shown the need and ability to rapidly scale eCR implementation in healthcare organizations across the country. To continue to modernize how public health surveillance is conducted using data now electronically available in electronic health records, public health must strategically modernize the approach, use of technology and have a extensible infrastructure to accept, process and distribute the electronic reports to all public health agencies. This eCR project should support electronic case reporting for all reportable conditions and work together with CSTE, CDC, EHR Vendors, healthcare organizations and public health agencies to scale nationwide this modern approach to public health reportable condition reporting.

Activities: Describe activities to be carried out in this project as well as their associated deliverables:

Provide strategic leadership for eCR in the areas of architecture, technical, and policy needs.

Technical assistance

• Supporting integration of eICRs and RRs with surveillance system

• Mapping tools – for Rhapsody and Mirth

• Technical consultation and advice

• Methods to route data within PHA to appropriate program

• Matching methods for ELR and eCR

• Methods to ensure most recent eICR is integrated into system and not pervious eICRs

• Facilitate Data Quality Subgroup

Onboarding new healthcare organizations

• Establish new connection

• Support test and production environments

• Validation of test messages

• Review of approved policy frameworks approach

• Filtering and authorization of reporting sites

• Go live monitoring

EHR Vendor Engagement

- EHR Recruitment o Outreach and communication with new vendors o eCR Now COVID-19 Challenge

- Testing and support for new EHR product capability o Communication of key requirements o eICR testing and validation o “sandbox” testing of new capabilities o Final sign off for implementation in client sites

Processing (production) monitoring

• Incoming feeds operational

• Error queue review and resolution

• RR delivery confirmations to healthcare

• eICR and RR delivery confirmations to PHAs

• Replays if necessary

RCKMS decision support service development, deployment, operations, and hosting

• New AIMS infrastructure to support microservices deployment

• Continue development of decision support service as new requirements are identified

• Routine deployments – current one is a complete re-architecture to microservices to address performance issues

• Urgent patches or updates

• Monitoring and troubleshooting of performance

Modernize connectivity approach by maintaining and operating a HISP for connectivity to eHealth Exchange hub and ensure Electronic Healthcare Network Accreditation Commission (EHNAC) accreditation

Advance and maintain policy and trust frameworks for eHealth Exchange (eCR use case), Carequality, CommonWell and DirectTrust

Maintain and operate creation and distribution of triggering guidance – eRSD (electronic Reporting and Surveillance Distribution) Service

• Subscription provides notification (and push) of updates

• FHIR Distribution of Trigger Value Sets and Trigger use specification (timing, etc.)

• Trigger value sets provided by RCKMS Content Team

• Updates have to be made as new trigger types are added

Establish capability for distributing rules for healthcare

• Reduce platform volume for not reportable

• “all lab results” is not sustainable – costs and infrastructure

• CQL distribution of next level rules for filtering to reduce traffic – take out not needed negative lab tests

• Could be path for distributing all logic (if healthcare was ever capable)

• In eCR Now app - other EHRs do not have implementation ability yet

Maintain, update and operate validation service – online and inline

• Online used by implementers to test message structure and minimum content

• Inline used when processing eICRs on AIMS

• Ensure validation for all standards versions in use

• Development and maintenance of Schematron for use in validator

Develop, maintain, update and support eCR Standards

• eICR CDA (reballoting Jan 2021 – to be 3.0 when published)

• RR CDA

• eCR FHIR (includes eICR, RR and trigger specifications) (reballoting Jan 2021, ensure publication)

• CDA and FHIR transforms

• Data element analysis – comparing to other data streams and mappings to eICR and RR

Create and maintain Website and other communication materials
