About this file

This document is a Past Performance Questionnaire that requests an assessment of a contractor's performance in providing janitorial services at the Bay Pines National Cemetery. The questionnaire covers various aspects of the contractor's performance, including their ability to successfully provide the required services, the qualifications and experience of their personnel, adherence to the schedule and statement of work, overall management, communication, and staffing. Respondents are asked to rate the contractor's performance on a scale from 1 (Unsatisfactory) to 5 (Exceptional) for each of these criteria, and to provide a narrative explanation for any ratings of 1 or 6. The document also includes a brief overview of the related federal contract opportunity, which is for Janitorial Services at the Bay Pines National Cemetery, with a base period and four option renewals. The contract is set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs).
